摘要
目的探讨椎管肿瘤诊断中MRI的临床应用。方法选择收治的40例椎管肿瘤患者,应用MRI诊断,分析临床特点。结果 40例椎管肿瘤患者中,脂肪瘤3例,星形细胞瘤6例,转移瘤7例,室管膜瘤5例,脊膜瘤7例,血管母细胞瘤1例,神经纤维瘤2例,神经鞘瘤9例。依据肿瘤所处位置分类:(1)髓外硬膜下肿瘤:检出18例,包括脊膜瘤,神经纤维瘤,神经鞘瘤,脊髓受压变形均可显示,并移位对侧,蛛网膜下腔在肿瘤对侧变窄,肿瘤侧增宽,临床将其定义为硬膜下征。瘤体在增强扫描后明显强化。(2)髓内肿瘤:共12例,包括血管母细胞瘤、室管膜瘤、星形细胞瘤,多在颈胸段,以脊髓境界不清,程度不等增粗为主要表现。(3)髓外硬膜外肿瘤:共10例,包括脂肪瘤及转移瘤,硬膜外肿块影6例,呈不规则形态。结论 MRI软组织分辨力较高,具无创性,可任意扫描,与靶向造影剂(Gd-DTPA)结合增强扫描,对椎管内病变的结构、位置及相邻关系可直接显示,准确定性和定位诊断病变,为最理想的影像学检查方法。
Objective To investigate the clinical application of MRI in the diagnosis of intraspinal tumor. Methods 40 intraspinal tumor patients were admitted to our hospital, patients were diagnosed according to MRI, clinical character were analyzed. Results Among 40 patiens, 3 cases of lipoma, 6 cases of astrocytoma, 7 cases of metastases, 5 ependymoma, meningioma in 7 cases, 1 case of hemangioblastoma, 2 cases of neurofibroma, 9 cases of schwannoma. According to the tumor location classification:(1) subdural extramedullary tumors: 18 cases were detected, including meningioma, neurofibroma, neurilemmoma, spinal cord compression deformation can be displayed, and the shift of the side, the side of the subarachnoid space narrowing in the tumor, tumor side broadening, clinical will be defined as subdural syndrome. The tumor enhancement in enhanced scan.(2) 12 cases of intramedullary tumors, including astrocytoma, ependymoma, hemangioblastoma, in the cervicothoracic spinal cord, the state is not clear, different degrees of thickening is the main performance.(3) The intradural extramedullary tumor: a total of 10 cases, including 6 cases of lipoma and transfer Island, epidural mass, irregular shape. Conclusion MRI has high soft tissue resolution, non-invasive, can be arbitrary scanning, combined with Gd-DTPA enhanced scanning, structure, position and adjacent relationship of intraspinal lesions can be directly display, accurate qualitative diagnosis and location of lesion, examination method is the most ideal image.
