摘要
目的探讨纤维桩核恢复后与邻牙联冠修复和纤维桩核恢复后单冠修复治疗无牙本质肩领残根的临床效果,为无牙本质肩领残根的临床治疗提供经验。方法将2007年1月—2008年3月间在口腔科进行治疗的无法形成牙本质肩领的前牙残根患者170例,分为A组和B组。其中,A组患者95例,采用纤维桩核恢复后与邻牙联冠修复进行治疗;B组患者75例,采用纤维桩核恢复后单冠修复进行治疗。术后2组患者均随访5年,并对两种手术的修复效果进行比较。2组数据采用SPSS 16.0统计软件进行统计分析。结果 2组患者术后随访5年,共有6名患者失访。A组成功随访90名,B组成功随访72名。A组修复成功88例,有2例患者发生了脱落,A组脱落率为2.2%,而B组修复成功55例,脱落17例,脱落率为23.6%,2组脱落率比较,差异有统计学意义(P<0.05)。2组不同牙位的脱落率比较,差异无统计学意义(P>0.05)。结论相比于纤维桩核恢复后单冠修复,纤维桩核恢复后与邻牙联冠修复可有效治疗无法形成牙本质肩领的前牙残根,牙齿脱落率低,临床建议采用纤维桩核与邻牙联冠修复进行无法形成牙本质肩领的前牙残根的治疗。
Objective To compare the clinical efficacy of combined crown (connected to adjacent teeth ) and single crowns applied to the restoration for residual roots with no ferrule after fiber post cementation and core buildup, and provid reference for the treatment of anterior residual root which couldn' t prepare the dentine collar. Methods 170 patients with anterior residual root which could not prepare the dentine collar in our department from January 2007 to March 2008 were selected and divided into group A and group B by random number methods. 95 patients in the group A were restored with combined crown connected to adjacent teeth after fiber post cementation and core buildup, while 75 patients in the group B were restored with single crowns. All patients were followed up for 5 years, and the effects of two prosthetic meth- ods were compared. The datas of two groups were statistical analyzed by SPSS 16.0 statistical software. Results 6 pa- tients were lost to follow-up. There were still 90 patients in group A, and 72 in group B. 88 cases in the group A were suc- cessful prepared,the crown in 2 cases falled off with a rate of 23.6% in the group A. 55 cases in the group B were suc- cessful prepared,he crown in 17 cases failed off with a rate of the group B. The difference on falling off rate between the two groups was significant(P 〈0.05). The difference on falling off rate of various tooth between the two groups was not significant(P 〉 0.05). Conclusion When compared with single crowns, the connected crowns with adjacent teeth after fiber post cementation and core buildup can effectively prepare anterior residual root which couldn' t prepare the dentine collar. This method should be recommended.
