摘要
目的探讨早期肠内营养和肠外营养支持对早期胃癌切除术后患者的影响。方法选取早期胃癌切除术后患者120例,按照随机数字表法将患者随机分为治疗组和对照组,每组各60例。治疗组患者给予肠内营养支持,对照组患者给予肠外营养支持,干预周期为7 d。结果与治疗组患者比较,营养支持后对照组患者血清总胆红素、丙氨酸氨基转移酶和门冬氨酸氨基转移酶值明显升高,差异有统计学意义(P<0.05)。营养支持后,两组患者血清前白蛋白、血清血蛋白、CD+3和CD+4值明显升高,差异均有统计学意义(均P<0.05)。随访3个月,治疗组患者的切口感染、心胸腔积液、肺部感染、吻合口漏等并发症的总发生率明显小于对照组患者,差异有统计学意义(P<0.05)。结论肠内营养支持能促进早期胃癌切除术后患者免疫功能和机体营养状况的恢复,对患者的肝功能损害小,安全性更好,值得临床推广应用。
Objective To investigate the effects of early enteral nutrition and parenteral nutrition support on early gastric resection patients. Methods 120 patients with early resection gastric cancer were enrolled into this study and randomly divided into treatment group and control group, 60 patients in each group. Patients in the treatment group were received enteral nutrition, and patients in the control group were given the parenteral nutrition, the intervention periods were seven days. Results After nutritional support, the ALT and AST values were significantly increased in the control group when compared with the treatment group, the difference was significant ( P 〈 0. 05 ). The serum PALB and ALB values, CD3+ and CD4+ values were increased significantly after nutritional support in the two groups, the difference was significant ( P 〈 0. 05 ). After three-month followed-up, the overall incidence of complications of incision infection, heart plcural effusion, pulmonary infection, anastomotie leak of the treatment group were significantly less than that in the control group ( P 〈 0. 05 ). Conclusions Enteral nutrition can promote the recovery of early postoperative gastric patients' immune function and nutritional status, its damage of liver function was small, it had better safety and should be widely applied.
