右美托咪定对行全身麻醉下经腹全子宫切除术患者痛觉过敏的影响 被引量:2

摘要 目的探讨右美托咪定对行全身麻醉下经腹全子宫切除术患者痛觉过敏的影响。方法将择期行全身麻醉下经腹全子宫切除术患者72例随机分为观察组与对照组,每组36例。麻醉前30 min,观察组持续静脉滴注右美托咪定0.5μg/kg,对照组持续静脉滴注等量的生理盐水。2组均输注瑞芬太尼及异丙酚进行麻醉诱导及维持,手术后所有患者在麻醉恢复室内观察2 h。记录并比较2组术后VAS疼痛评分、恢复室内静脉注射吗啡用量及不良反应发生情况。结果与对照组比较,观察组麻醉停药后第1次出现VAS疼痛评分≥4分的时间明显延长,注射镇痛剂后VAS评分降至4分以下所需时间明显缩短,恢复室内静脉注射吗啡用量明显减少,差异均有统计学意义(P均<0.05);2组不良反应发生率比较差异无统计学意义(P>0.05)。结论对行全身麻醉下经腹全子宫切除术患者静注右美托咪定能有效缓解术后疼痛,减少术后镇痛药的用量,并且能够减轻麻醉药所致的痛觉过敏,值得临床上推广应用。 Objective It is to explore the effect of dexmedetomidine on hyperalgesia of the patients undergoing transabdominal hysterectomy under general anesthesia. Methods 72 patients undergoing selective transabdominal hysterectomy under general anesthesia were randomly divided into observation group and control group, each group had 36 cases. 30 rain before anesthesia, the observation group was given dexmedetomidine 0.5 v.g/kg by continuous intravenous drip, while the control group was given the same volume of normal saline by continuous intravenous drip. Both the groups was given fentanyl and propofol for anesthesia induction and maintenance, all of the patients were observed for 2h after the operation in the PACU. Postoperative VAS pain scores, the dosage of intravenous injection of morphine in PACU and occurrences of adverse reactions of both groups were recorded and compared. Results Compared with the control group, after discontinuation of anesthesia the first time with VAS pain scores of 4 points or more was extended obviously, the time with VAS scores decreasing to the line under 4 points after injection of analgesics was significantly shortened, the consumption of morphine by intravenous drip in PACU decreased obviously in the observation group( P all 〈 0.05 ) , there was no significant difference in the occurrence rate of side effect between the two groups ( P 〉 0.05 ). Conclusion For the patients undergoing selective transabdominal hysterectomy under general anesthesia, dexmedetomidine by continuous intravenous drip can effectively relieve postoperative pain, reduce the postoperative consumption of analgesics, and can reduce hyperalgesia caused by anaesthetic, it is worthy of wide application in clinic.
