运动症状和嗅觉及经颅多普勒超声在帕金森病诊断中的应用价值 被引量:8

摘要 目的探讨运动症状评估、嗅觉检测、经颅多普勒超声以及三者联合在帕金森病诊断中的应用价值。方法收集帕金森病患者50例(帕金森病组),并选取性别相同、年龄相当的非帕金森病患者50例(对照组),分别采用帕金森病统一评定量表、T&T标准嗅觉检测、经颅多普勒超声检测技术评估非对称运动、嗅觉功能和黑质回声,分析嗅觉障碍和黑质强回声的影响因素,并分别计算3种方法以及它们联合诊断帕金森病的敏感性、特异性和阳性预测值。结果帕金森病组非对称运动症状、嗅觉障碍、黑质强回声比例较对照组明显增高(P<0.05,P<0.01);嗅觉障碍与帕金森病程、疾病严重程度呈正相关;运动症状评估、嗅觉检测、经颅多普勒超声检查的敏感性、特异性和阳性预测值分别为80.0%,78.0%,78.4%;58.0%,74.0%,69.0%;90.5%,81.3%,80.9%。而联合用于帕金森病诊断的敏感性、特异性和阳性预测值分别为38.1%,97.9%,94.1%。结论运动症状、嗅觉检测、经颅多普勒超声检查有利于帕金森病诊断,而将三种方法联合应用则可提高诊断的特异性。 Objective To study the application of motor symptoms,olfactory test and transcranial ultrasonography in diagnosis of Parkinson's disease(PD).Methods Fifty PD patients served as a PD group,and 50age-and gender-matched non-PD individuals served as a control group in this study.Their motor asymmetry,olfactory function and substantia nigra(SN)echogenicity were assessed according to the UPDRSⅢ,TT olfactometry and transcranial ultrasonography respectively.Hyposmia and SN hyperechogenicity(SN+)'s influencing factors were analyzed.The sensitivity,specificity and positive predictive value of combined motor symptoms,olfactory test and transcranial ultrasonography for diagnosis of PD were calculated respectively.Results The motor asymmetry,hyposmia and SN+ were significantly higher in PD group than in control group(P〈0.05,P〈0.01).The olfactory dysfunction was positively related with the course and severity of PD.The sensitivity,specificity and positive predictive value of motor symptoms,olfactory test and transcranial ultrasonography were 80.0%,78.0%,78.4%;58.0%,74.0%,69.0%;and 90.5%,81.3%,80.9% respectively for the diagnosis of PD.The sensitivity,specificity and positive predictive value of combined motor symptoms,olfactory test and transcranial ultrasonography were38.1%,97.9%,94.1%respectively for the diagnosis of PD.Conclusion Motor symptoms,olfactory test and transcranial ultrasonography contribute to the diagnosis of PD and their combination improves the diagnostic specificity.
