摘要
目的 总结囊袋松解术（CBRS）应用于白内障超声乳化术后囊袋收缩综合征（CCS）的临床效果.方法 回顾性病例研究.对19例（19眼）白内障超声乳化术后CCS者行CBRS,其中12眼行放射状切开松解,7眼行二次连续环形撕囊法.松解术后观察术眼UCVA和BCVA,并和松解术前UCVA进行比较.裂隙灯显微镜观察前囊口透明区大小,前、后囊膜情况及IOL位置等.并了解有无主观症状如眩光或单眼复视等.术后随访6个月.松解术前后视力比较采用符号秩和检验.结果 所有术眼术后视力较术前均有不同程度的提高,眩光和单眼复视现象消失,视物清晰.手术前后视力差异有统计学意义（U=5.435,P＜0.01）.裂隙灯显微镜下见囊袋收缩松解,前囊口透明区面积扩大,IOL位置稳定居中.结论 对于CCS者,CBRS可改善视力,提高视物舒适度,保持IOL位置稳定居中.
Objective To study the clinical effectiveness of capsular sac relaxing surgery （CBRS） for capsular contraction syndrome （CCS）.Methods In a retrospective study,19 patients （19 eyes） who presented with CCS after cataract phacoemulsification with IOL implantation underwent CBRS.Eleven patients （11 eyes） underwent surgery for a radial incision relaxing procedure and 8 patients （8 eyes） underwent a second continuous curvilinear capsularrhexis （CCC）.Uncorrected visual acuity and best corrected visual acuity of the surgically treated eyes were checked postoperatively and compared with their preoperative uncorrected vision.A slit-lamp biomicroscope was used to observe the change in the size of the transparent zone of the anterior capsule holes,the condition of the anterior and posterior capsules,and the IOL position.Patients were also assessed for whether or not they experienced subjective symptoms such as glare or simple blur.Patients were followed up for 6 months.Results There were different degrees of improvement in visual acuity postoperatively in all surgically treated eyes （Wilcoxon signed rank test,U=5.435,P〈0.01）.Glare or simple blur disappeared and patients could see more clearly postoperatively.Slit-lamp examination showed that the capsular sac contraction had relaxed,the sizes of the transparent zones of the anterior capsule were enlarged and the position of the IOL was stable and centered in all surgically treated eyes.Conclusion CBRS is an effective method for patients with CCS,visual acuity improved,subjective symptoms disappenred,the position of IOL stabled.
