左房容积指数结合血浆脑钠肽对左室正常构型的高血压患者心肌损伤的评估 被引量:1

Left Atrial Volume Index Combined with Plasma BNP for Evaluation on Myocardial Injury in Hypertension Patients with Normal Left Ventricular Geometry

摘要 目的评价左房容积指数(LAVI)结合血浆脑钠肽(BNP)在左室正常构型高血压患者心肌损伤评估中的价值。方法选取左室正常构型原发性高血压患者110例。A组55例为LAVI正常组,B组55例为LAVI增高组,N组选取40例血压正常者作为对照,所有入选者二维超声左室射血分数(LVEF)>50%。收集、分析比较超声各参数与血浆BNP的相关性。结果 1高血压A、B组较N组BNP增高,B组BNP较A组增高,均有显著差异性(P<0.05);2超声各参数比较:高血压A、B组较N组左室心脏舒张功能减低,差异有统计学意义(P<0.05),但A、B组之间左室舒张功能差异无统计学意义;3超声各参数中LAVI与BNP存在正相关(r=0.573,P<0.01)。结论左室正常构型的早期高血压患者左室舒张功能减低,在左室肥厚之前已经存在心肌损伤。 Objective To evaluate the extent of myocardial injury in the hypertension patients with normal left ventricular geometry with left atrial volume index(LAVI)combined with plasma BNP. Methods All 1 10 hypertension patients with normal left ventricular geometry were divided into groups.Fifty-five hypertension patients with normal LAVI was enrolled in group A,Fifty-five hypertension patients with decreased LAVI was enrolled in group B,and 40 non-hypertensive patients were enrolled in the control group,whose relationship between that sufferers′LVEF 〉 50% with Ultrasound.The echocardiographic parameters and plasma BNP relevance were analysed. Results ① Compared with the control group,the plasma BNP of Group A and B were increased(P 〈0.05),respectively.Group B compared with Group A, the Plasma BNP increased significantly too(P 〈0.05 ).②The left ventricular diastolic function of group A and B were decreased which compared with N group(P 〈0.05 ),respectively;but there was no significant difference of the left ventricular diastolic function in group A and B.③The arameters of Ultrasound,LAVI was positively correlated with plasma BNP (r =0.573,P 〈0.01 ). Conclusion The left ventricular diastolic function can decrease the hypertension patients with normal left ventricular geometry.Hypertensive induced the cardiac injury before the changes of left ventricular hypertrophy.
