摘要
慢性粒细胞白血病(CML)是一种发生在骨髓造血干细胞的恶性克隆性骨髓增殖性疾病。具有自我更新和自我复制能力的造血干细胞在体内增殖、分化、迁徙、转移保持平衡,主要归功于稳定的造血微环境,即"小生境",它包括微血管系统、基质干细胞、细胞外基质和多种细胞因子。而骨髓基质细胞分泌的趋化因子CXCL12及造血干细胞表面受体CXCR4组成的生物轴(CXCL12-CXCR4 biological axis)是调节造血干细胞生物学行为的重要影响因子。现代研究发现,"活血化瘀"类中药不仅可以干预骨髓造血干细胞的增殖分化,而且影响"干细胞循环",就其循环过程来看,与活血化瘀法"祛瘀血"层面的观点相吻合;从循环结果来看,与活血化瘀法"生新血"层面的观点相吻合。文章意在探讨造血干细胞小生境中CXCR4/CXCL12轴在慢性粒细胞白血病可能存在的特殊生物学机制,从造血干细胞生物学行为层面探索慢性粒细胞白血病,并为慢性粒细胞白血病发病机制的研究找到可能存在的新突破点——造血微环境,进一步为以后临床及实验研究提供有价值的理论依据。
Chronic myeloid leukemia (CML) is a kind of malignant clonal myeloproliferative disease which happens in the bone marrow hematopoietic stem cells. The hematopoietic stem cells with self - renewal and self - replicating ability remains con- stant in the size and number in the stem cells pool, mainly due to hematopoietic microenvironment which maintained its stable, it is also called niche. The regulating factor in the niche is the main members participating in this activity. Modem research found that Chinese herbology which can promote circulation and remove stasis can not only intervene in the proliferation and differentia- tion of bone marrow hematopoietic stem cell, but also affect the stem cell cycle. Considering the progress of the cycle,it agrees with the view removing blood stasis. Considering the result of the cycle, it agrees with the view of giving birth to the new blood. This ar- ticle is intended to explore the specific biological mechanisms of CXCR4/CXCL12 axis in the hematopoietic stem cell niche in chronic myeloid leukemia. It will explore chronic myeloid leukemia from the level of the biological behavior of hematopoietic stem ceils and find a possible breakthrough in point - hematopoietic microenvironment for the pathogenesis of chronic myeloid leukemia cell research. And it further provides valuable theoretical basis for future clinical and experimental studies.
