甲亢及甲减患者血清瘦素水平与甲状腺激素相关性研究 被引量:10

Correlation between serum leptin levels and thyroid hormone levels in patients with hyper- and hypothyroidism

摘要 目的研究甲状腺功能亢进(甲亢)、甲状腺功能减退(甲减)患者及正常人中血清瘦素水平及其与甲状腺激素的关系。方法选取正常体质量的甲亢患者29例(甲亢组)、甲减患者22例(甲减组)、健康体检合格者35名(正常对照组)。采用电化学发光法检测血清FT3、FT4和TSH;采用放射免疫法检测血清瘦素水平。结果甲亢患者血清瘦素水平(10.01±5.67)ng/ml高于对照组(5.36±2.11)ng/ml及甲减组(5.40±1.99)ng/ml(P均<0.01);甲减组瘦素水平与对照组比较差异无统计学意义(P>0.05)。Pearson相关分析显示,瘦素与FT3、FT4呈正相关(FT3:r=0.728,P<0.01;FT4:r=0.776,P<0.01),而与BMI和TSH无显著相关性(P>0.05)。结论血清瘦素水平在甲亢患者中高于正常,与FT3、FT4呈正相关,提示瘦素的表达与甲状腺功能状态具有一定的相关性。 Objective To study the correlation between serum leptin levels and thyroid hormones concentration in patients with hyper-and hypo-thyroidism,and healthy subjects.Methods Twenty nine hy-perthyroid patients with normal weight (hyperthyroidism group),22 hypothyroid cases (hypothyroidism group) and 35 healthy controls (control group)were recruited in this investigation.Serum thyroid-stimulating hormone (TSH),free triiodothyronine (FT3 )and free thyroxine (FT4 )levels were measured with electrochemilumines-cent immunoassay (ECLIA)method.Serum leptin level was assessed by radio-immunoassay (RIA).Results The serum leptin level in hyperthyroid patients was (1 0.01 ±5.67)ng/mL,significantly higher compared with (5.36 ±2.1 1 )ng/mL in healthy controls and(5.40 ±1 .99)ng/mL in hypothyroid patients(both P 〈0.01 ). The serum leptin levels did not significantly differ between the hypothyroid and control groups (P 〉0.05). Pearson correlation analysis revealed that serum leptin level was positively correlated with FT4 and FT3 levels (FT3:r =0.728,P 〈0.01 ;FT4:r =0.776,P 〈0.01 ),but had no correlation with BMI and TSH.Conclu-sions Serum leptin level in hyperthyroid patients was higher than that in normal controls.Leptin level was positively correlated with FT3 and FT3 levels,suggesting that leptin expression was associated with thyroidal function to certain extent.
