摘要
目的评价精囊镜检查在顽固性血精诊疗中的应用价值。方法回顾性分析我院2010--2012年收治的9例顽固性血精病人的临床资料，病人均于MRI检查后使用WOLFF6／7．5细输尿管硬镜进行精囊内检查。结果9例病人手术过程顺利，手术时间17～37min，平均（26．1±9．5）min。术中发现精囊慢性炎症6例，精囊内结石2例，射精管梗阻扩张1例。2例囊腔内有充血改变，有散在出血点，活组织病理检查示炎症改变。给予相应治疗后随访半年，7例未再出现血精；2例精囊慢性炎症病人再次出现血精，再次行精囊镜检查并给予相应治疗后未再复发。结论精囊镜检查可以在一定程度上明确血精的病因，并针对病因给予相应的治疗，手术风险小，并发症少，对治疗顽固性血精有一定的价值。
Objective To evaluate the value of seminal vesiculoscopy in diagnosis and treatment of refractory hemospermia. Methods From 2010 to 2012, a total of nine patients with refractory hematospermia were treated in our department, after MRI examination, all the patients underwent seminal vesiculoscopy using F6/7.5 rigid ureteroscope. A retrospective review was done for the data. Results The operations for the nine patients were smoothly completed, operating time being （average） 17--37 （26.1± 9.59） rain. The findings at surgery were： chronic inflammation of seminal vesicles in six cases, calculus of seminal vesi- cle in two cases, and one of obstruction and extension of ejaculatory ducts. Hyperemia inside the capsular space with scattered bleeding points were found in two cases, a biopsy was done and inflammation confirmed. A corresponding therapy was carried out and followed six months later, no recurrence of hemospermia was noted in seven cases; and two of chronic inflammation recurred, after a re-examination with seminal vesiculoscopy and further therapy, hemospermia did not occur again. Conclusion Seminal veslculoscopy can, in a certain degree, identify the cause of hemospermia, and provide an appropriate therapy. The procedure risk is small with few complications, there is a certain value for treatment of refractory hematospermia.
