摘要
目的:分析及探讨晚期肝癌并发症与中医证型之间的关系。方法:采用回顾性研究,对48例晚期肝癌患者的临床证候及并发症进行频数统计,对证候进行聚类分析,归纳中医证型与并发症之间的关系。结果:晚期肝癌的证型主要有热毒瘀肝型、肝郁脾虚型、肝肾阴虚型、肝胆湿热型及脾虚湿困型。48例死亡肝癌患者中并发肝性脑病35例,31例出现继发感染,并发消化道出血20例,并发肝肾综合征10例,自发性低血糖和胃肠道梗阻各3例。其中热毒瘀肝型的并发症以肝性脑病、继发感染和消化道出血为主,肝郁脾虚型的患者肝性脑病发生率高达100%;自发性低血糖的发生率在肝肾阴虚型中较高;脾虚湿困型易发生消化道出血和胃肠道梗阻。探讨中医证型与并发症之间的关系,可以为提早做出干预措施以减少并发症、延长患者的生存期提供依据。
Objective：To analyze and discuss the relationship between late complications of liver cancer and the traditional Chinese medicine syndrome type. Methods：The retrospective study method was used.And frequency statistics was made according to 48 cases of patients with advanced liver cancer clinical syndrome and complication,cluster analysis of TCM syndrome,summarized the relationship between the TCM syndrome type and complications. Results：Justification of advanced liver cancer are mainly internal toxic heat stasis liver,liver depression and qi stagnation type,liver and kidney Yin deficiency type,liver syndrome and wet trapped pixu type.48 patients died of liver cancer patients complicated with hepatic encephalopathy in 35 cases,secondary infection in 31 cases,complicated with gastrointestinal hemorrhage in 20 cases,10 cases complicated with hepatorenal syndrome,Spontaneous hypoglycemia and gastrointestinal obstruction with 3 cases in each. Complications of toxic heat stasis liver type with hepatic encephalopathy,secondary infection and gastrointestinal bleeding,patients with liver depression and qi stagnation type of hepatic encephalopathy incidence is 100%;the incidence of spontaneous hypoglycemia is higher in the liver and kidney Yin deficiency type;Wet pixu trapped prone to gastrointestinal bleeding and gastrointestinal obstruction.Conclusion：Explore the relationship between TCM syndrome and complications that can be made for early interventions to reduce complications and prolong survival of patients provide evidence.
