三种超声斑点追踪技术评价正常人左心室心肌应变 被引量:13

Three different speckle-tracking techniques in assessment of left ventricular myocardial strains in healthy subjects

摘要 目的探讨三种超声斑点追踪成像(STI)技术[包括二维STI(2D-STI)、三平面STI(3P-STI)及四维应变(4Dstrain)]评价健康成人左心室心肌应变的价值。方法采用2D-STI、3P-STI及4D-strain技术对141名健康成人行心肌运动分析,测量左心室整体及各节段的纵向、圆周、径向及面积应变,比较3种方法所测指标的差异及相关性。结果 2DSTI和3P-STI的整体纵向应变(GLS)及同一水平各节段纵向峰值应变(LS)比较差异无统计学意义(P>0.05),但均高于4D-strain(P<0.05)。2D-STI在同一水平前间隔及后间隔圆周峰值应变(CS)高于4D-strain,其余各节段均低于4Dstrain,而在同一水平各节段径向峰值应变(RS)均低于4D-strain(P均<0.05)。不同水平及同一水平不同节段面积应变(AS)比较,差异均无统计学意义(P>0.05)。4D-strain存储图像和分析图像所用时间短于2D-STI(P<0.05)。2D-GLS与3P-GLS和4D-GLS有相关性(r=0.718,P<0.001;r=0.397,P=0.006)。三种方法观察者间和观察者内一致性均较好。结论 2D-STI和3P-STI技术获得LS具有较好的一致性。4D-strain是一种评价左心室心肌纵向、圆周、径向和面积应变的新方法,具有快捷、全面、重复性好的优点,对定量评价室壁运动具有很好的临床应用价值。 Objective To explore the value of three different speckle-tracking techniques including two-dimensional speckle tracking imaging(2D-STI),tri-plane speckle tracking imaging(3P-STI)and four-dimensional strain(4D-strain)in evaluation of left ventricular(LV)myocardial strains in healthy subjects.Methods Totally 141 healthy subjects underwent myocardial motion analysis for measuring LV longitudinal peak systolic strain,circumferential peak systolic strain,radial peak systolic strain and area strain by 2D-STI,3P-STI and 4D-strain,then the differences and correlations were compared among three methods.Results There was no significant difference in global and segmental LV longitudinal peak systolic strain(LS)between 2D-STI and 3P-STI(P〉0.05),but were significantly higher than those of 4D-strain(P〈0.05).In the same level of anteroseptal and inferoseptal,the circumferential peak systolic strains(CS)of 2D-STI were higher than those of 4D-strain,but other segments were lower than those of 4D-strain,and in the same level of different segments,the radial peak systolic strain(RS)of 2D-STI was lower than those of 4D-strain(P〈0.05).Area strain in different levels and segments had no statistical significance in 4D-strain(P〉0.05).The time for acquisition and analysis of the images by 4Dstrain were shorter than those by 2D-STI(P〈0.05).2D-GLS correlated with 3P-GLS and 4D-GLS(r=0.718,P〈0.001;r=0.397,P=0.006).There was high consistency between inter-and intra-observer agreement among three methods.Conclusion There is good consistency for longitudinal strain between 2D-STI and 3P-STI.4D-strain is a convenient,comprehensive,reproducible and a new method to evaluate longitudinal strain,circumferential strain,radial strain,and area strains of LV,which have great clinical application value in quantitative analysis ventricular movement.
