摘要
目的:探讨乳腺癌中上皮钙黏蛋白(E-cadherin)和B细胞淋巴瘤基因2(Bcl-2)的表达及其临床意义。方法:使用免疫组织化学方法检测68例乳腺癌旁正常组织、41例纤维腺瘤组织和74例乳腺癌组织的E-cadherin和Bcl-2的表达水平,并分析乳腺癌患者各病理特征与E-cadherin和Bcl-2表达的相关性。结果:乳腺癌旁正常组织和纤维腺瘤组织中E-cadherin阳性率高于乳腺癌组织,Bcl-2阳性率低于乳腺癌组织,差异具有统计学意义(P均<0.05)。乳腺癌组织病理分级越高、TNM分期越严重、有淋巴结转移和复发的患者E-cadherin阳性率低,Bcl-2阳性率高;ER阳性患者E-cadherin阳性率高,Bcl-2阳性率高。组织病理分级低-中分化患者E-cadherin阳性率高于高分化患者,Bcl-2阳性率低于高分化患者,差异具有统计学意义(P均<0.05);TNM分期Ⅰ+Ⅱ期患者E-cadherin阳性率高于Ⅲ期,Bcl-2阳性率低于Ⅲ期,差异具有统计学意义(P均<0.05);有淋巴结转移患者E-cadherin阳性率低于无转移者,Bcl-2阳性率高于无转移者,差异具有统计学意义(P均<0.05);雌激素受体(ER)阴性患者E-cadherin、Bcl-2阳性率低于ER阳性者,差异具有统计学意义(P均<0.05);复发患者E-cadherin阳性率低于无复发者,Bcl-2阳性率高于无复发者,差异具有统计学意义(P均<0.05);Bcl-2的表达与E-cadherin的表达存在负相关性(r=-0.638,P<0.05)。结论:乳腺癌组织中E-cadherin表达降低,Bcl-2表达升高,二者与乳腺癌组织病理分级、TNM分期、淋巴结转移、ER和复发有关,E-cadherin和Bcl-2的表达存在相关性。
Objective:To investigate the expression of epitlielium cadherin (E-cadherin) and B cell lymphoma gene 2 (Bcl-2) in breast cancer and its clinical significance.Methods:E-cadherin and Bcl-2 expression levels were detected by immunohistochemical method in 68 cases of breast cancer tissues,41 cases of fibroadenoma and 74 cases of breast cancer,and to analyze the correlation between the pathological features with E-cadherin and Bcl-2 expression in breast cancer patients.Results:The positive rate of Bcl-2 and E-cadherin in breast cancer and breast cancer tissues compared with adjacent normal tissue and breast fibroadenoma tissues (P < 0.05),the difference was statistically significant;The higher the histopathological grading,more serious TNM staging,lymph node metastasis and recurrence patients with E-cadherin positive rate was low,the positive rate of Bcl-2 was high.The positive rate of ER positive patients with high E-cadherin and Bcl-2 positive rate.Histopathologic grading in patients with low-middle differentiation the positive rate of E-cadherin and Bcl-2 compared with high differentiation(P < 0.05),the difference was statistically significant;Comparison of TNM stage Ⅰ + Ⅱ in patients with Ⅲ stage the positive rate of E-cadherin and Bcl-2 Ⅲ (P < 0.05),the difference was statistically significant;Lymph node metastasis patients the positive rate of E-cadherin and Bcl-2 compared with and without metastasis (P < 0.05),the difference was statistically significant;Estrogen receptor(ER) negative patients the positive rate of Bcl-2 and E-cadherin compared with positive(P < 0.05),the difference was statistically significant;Recurrence in patients the positive rate of Bcl-2 and E-cadherin compared with no relapse(P < 0.05),the difference was statistically significant;There is a negative correlation between the expression of E-cadherin of Bcl-2 (r =-0.638,P < 0.05).Conclusion:Reduced expression of E-cadherin in breast cancer,the expression of Bcl-2 increased,they were correlated with breast cancer TNM staging,lymph node metastasis,ER and recurrence,expression of E-cadherin and Bcl-2 was correlated.
