摘要
目的分析高尿酸血症与老年脑梗死的相关性。方法选择2008年1月~2010年10月来我院就诊,诊断为新发脑梗死的老年患者164例作为脑梗组,选取同期160例老年健康体检者作为对照组,对比分析两组患者的收缩压(SBP)、空腹血糖(FPG)、糖化血红蛋白(Hb A1C)、胆固醇(CHOL)、低密度脂蛋白胆固醇(LDL-C)、C反应蛋白(CRP)、纤维蛋白原(FIB)及血尿酸(SUA)水平的差异性。脑梗组按尿酸水平再分为高尿酸组、正常尿酸组,随访2年,观察两组脑梗死复发情况。结果脑梗组患者的CHOL、LDL-C、CRP、FIB及SUA水平、高尿酸比例均显著高于对照组,差异均有统计学意义(P〈0.05)。Logistic多因素回归分析可知LDL-C、CRP、FIB和SUA水平均可能是脑梗死的独立危险因素,且影响强度大小依次为SUA、FIB、CRP和LDL-C,差异均有统计学意义(P〈0.05)。高尿酸组脑梗死复发率明显高于正常尿酸组。结论高尿酸血症可能是老年脑梗死新的独立危险因素。
Objective To study the relationship between hyperuricemia and cerebral infarction in the elderly patients.Methods A total of 164 elderly patients newly diagnosed as cerebral infarction from January 2008 to October 2010 were selected as the cerebral infarction group,160 healthy elderly persons in the same period were selected as the control group,then differences of systolic blood pressure(SBP),fasting blood glucose(FBG),glycosylated hemoglobin(Hb A1C),cholesterol(CHOL),low density lipoprotein-cholesterol(LDL-C),C-reactive protein(CRP),fibrinogen(FIB) and serum uric acid(SUA) levels were compared.The cerebral infarction group was divided into hyperuricemia group 、normal uric acid group by SUA level.They were followed up for two years,and observed the recurrence of cerebral infarction. Results The CHOL,LDL-C,CRP,FIB and SUA levels,hyperuricemia rate in cerebral infarction group were all significantly higher than control group,the differences were statistically significant(P〈0.05). The LDL-C,CRP,FIB and SUA levels were all probably independent risk factors to cerebral infarction by the logistic regression analysis,and the effect intension of SUA was the best,then FIB,CRP and LDL-C,the differences were statistically significant(P〈0.05). Cerebral infarction relapse rate of hyperuricemia group was significantly higher than normal uric acid group.Conclusion Hyperuricemia is probably a new independent risk factor to cerebral infarction in the elderly patients.
