摘要
目的:分析外阴阴道假丝酵母菌对5种抗真菌药物的耐药性,为临床用药提供参考。方法:选取2013年1月—2014年12月间收治的外阴阴道假丝酵母菌病患者233例,根据原发性及复发性进行分组,原发性外阴阴道假丝酵母菌(VVC)感染患者(112例)为原发组,复发性外阴阴道假丝酵母菌(RVVC)感染患者(121例)设为复发组,分别取其阴道分泌物培养及药敏试验,分析氟胞嘧啶、两性霉素B、伏立康唑、伊曲康唑、氟康唑5种常用药对假丝酵母菌的敏感性。结果:培养的233例患者阴道分泌物中分离出白假丝酵母菌199株(85.41%),非白假丝酵母菌为34株(14.59%);药敏试验结果显示敏感性,两性霉素B>伊曲康唑>伏立康唑>氟胞嘧啶>氟康唑;氟胞嘧啶对非白假丝酵母菌的敏感性显著高于白假丝酵母菌(P<0.05),两性霉素B和氟康唑对白假丝酵母菌的敏感性显著高于非白假丝酵母菌(P<0.05)。结论:原发性外阴阴道假丝酵母菌以白假丝酵母菌为主,而复发性白假丝酵母菌则相对较少,在治疗上应针对性用药,确保疗效,减少复发。
Objective: To analyze the sensitivity ofvulvovaginal Candidia in 5 kinds ofantifungal drugs, and to pro- vide a reference for clinical medication. Methods: As the object of study were selected 233 patients with VVC among Feb. 2013 and Dec.2014 in Shenzhen Shajing Hospital Affiliated to Guangzhou Medical University. According to the pri- mary and recurrent group, primary vulvovaginal candidia (VVC) 112 patients were in primary group, recurrent vulvovaginal candidiasis (RVVC) 121 patients were in relapse group, respectively, and vaginal secretions were cultured and drug sensitive plate inoculation, statistical contrast sensitivity to 5-fluorocytosine, amphotericin B, fluconazole and voriconazole and itraconazole, five kinds of commonly used drugs were analyzed. Results: The culture of the vaginal discharge of 233 cases including Candida albicans in 199 cases, accounting for 85.41%, non Candida albicans accounted for only 14.59%; from the results of drug sensitivity test, the sensitivity to amphotericin B, itraconazole, voriconazole, fluconazole and flucytosine; fluorine cytosine sensitivity to non Candida albicans significantly higher than that of Candi- da albicans, with statistically significant difference (P〈0.05), amphotericin B, fluconazole susceptibility to Candida albi- cans was significantly higher than non Candida albicans, with significant difference (P〈0.05). Conclusion: Primary vul- vovaginal candidia pathogens were C. albicans, C. albicans and recurrent vulvovaginal candidia was relatively small. The treatment should be targeted drug to ensure curative effect and to reduce the recurrence.
