摘要
目的研究克龄蒙联合坤泰胶囊预处理对卵巢低反应患者卵巢储备功能及体外受精结局的影响。方法纳入首次体外受精-胚胎移植(IVF-ET)失败,欲行二次IVF-ET,且已证实为卵巢低反应患者100例。随机均分为A、B、C、D四组,A组不行预处理,B组行克龄蒙预处理,C组行坤泰胶囊预处理,D组行克龄蒙联合坤泰胶囊预处理。持续3个月经周期,于第四个月经周期第2天测定患者血清卵泡刺激素(FSH)、促黄体生成素(LH)、雌激素(E2)水平及卵巢动脉收缩峰值流速(PSV),评价患者卵巢储备能力。随访至体外受精结局,统计促性腺激素(Gn)用量、获卵数、受精率、优质胚胎数。结果 1预处理前后对比,B、C、D组血清FSH、LH均有明显下降,PSV均明显上升;预处理后,B、C组血清FSH、LH均明显低于A组,PSV均明显高于A组,同时D组血清FSH、LH明显低于B组及C组,PSV明显高于B组及C组,差异均有统计学意义(P<0.05)。2C、D两组Gn用量明显少于A组或B组;D组获卵数、优质胚胎数明显多于A、B、C三组,差异均具备统计学意义(P<0.05)。结论联合使用克龄蒙和坤泰胶囊有助于提升卵巢低反应患者卵巢储备功能,进而改善体外受精结局。
Objective To investigate the influence of Climen combined Kuntai capsule preconditioning on ovarian reserve function in patients with poor ovarian response,and to research the outcome of in vitro fertilization. Methods 100 cases of patients,who had failed in previous IVF- ET treatment,and were proved to be poor ovarian response patients. All the cases were included in the study. They all were willing to take another IVF- ET in this study. We randomly divided them into four Groups,named A,B,C and D. Group A underwent no pretreatment,Group B underwent Climen pretreatment,Group C underwent Kuntai Capsule precondition,while Group D received Climen combined Kuntai Capsule precondition. All preconditioning continued for 3 menstrual cycles,then in the second day of fourth menstrual cycle,we tested their serum follicule- stimulating hormone( FSH),luteinizing hormone( LH),estrogen2( E2) level and ovarian artery peak systolic velocity,which aimed to show patients' ovarian reserve function. We also follow- up to the outcome of in vitro fertilization,recorded the gonadotropin( Gn) dosage,the number of oocytes,fertilization rate,high quality embryo. Results 1Before and after pretreatment,the serum FSH,LH level of Group B,C and D were significantly decreased,while the PSV were significantly increased. After pretreatment,the serum FSH and LH level of Group B and C were significantly lower than that of group A and PSV were significantly higher than that of group A. In the meantime,the serum FSH and LH level of Group D were significantly lower than that of group B and C and PSV were significantly higher than that of group B and C. All differences above showed statistically significant( P〈0. 05). The Gn dosage of Group C and D were significantly less than that of Group A or B. The number of oocytes and high quality embryo of Group D were significantly more than that of Group A,B and C. All differences above showed statistically significant(P〈0. 05). Conclusion Combining Clime and Kuntai Capsule help to improve the ovarian reserve function in patients with poor ovarian response,thus improve the outcome of in vitro fertilization. This is worthy of promotion.
