摘要
目的探讨社区医院护理人员职业紧张反应、工作倦怠对其抑郁症状的影响,改善护理人员身心健康,降低抑郁的发生率。方法应用横断面调查方法,于2014年2-7月,随机抽取辽宁省300所社区医院工作半年以上的护理人员810名为调查对象,进行自填式问卷调查。用付出-回报失衡量表、工作倦怠量表以及流行病调查专用抑郁自评量表,通过多元层次回归分析方法来验证护理人员工作倦怠在职业紧张对抑郁症状中起中介作用。以护理人员的职业紧张为自变量,工作倦怠为中介变量,抑郁症状为因变量,构建护理人员建构职业紧张源、工作倦怠及抑郁症状关系的理论模型,同时通过结构方程模型来验证该模型。结果护理人员的最终模型拟合效果指标分别为:RMSEA=0.025、SRMR=0.048、TLI=0.900、CFI=0.9ll、IFI=0.912,本研究所建构的模型拟合效果良好。护理人员任务过重对抑郁症状的效应最大。结论护理人员工作倦怠在职业紧张源对抑郁症状影响中起中介作用;职业紧张源对抑郁症状的影响即有直接效应又有间接效应。
Objective To explore effects of the nurses' response of the occupational stress and burnout on their career depressive symptoms, in order to improve physical and mental health of the nurses and to reduce the incidence of depression of the nurses from community hospitals. Methods Cross-sectional survey was applied in Liaoning Province from February to July 2014, and 810 nurse were randomly selected as the object of this study. By pay-reward imbalance Scale, Maslach Burnout Inventory and Epidemiological investigations dedicated self-rating depression scale. The multiple linear regression analysis was applied to verify the effect of Personal strain in occupational stress affecting depressive symptoms. Results The final fitting index were: RMSEA=0.025, SRMR=0.048, CFI=0.911, TLI=0.900, IFI=0.912 in the nurses. It showed that fitting effect of theory model was good. Role boundary in the nurses were the most important factors associated with depressive symptoms. Conclusions Personal strain was a mediator in occupational stress affecting depressive symptoms. The direct and indirect effects were shown in occupational stress effecting depressive symptoms.
