摘要
目的:统计测量外侧半月板后根部损伤(Posterior Lateral meniscus root lesion,PLMRL)病例中半月板股骨韧带(Meniscofemoral ligament,MFL)完整型和MFL缺失型的外突发生率和外突值,探讨半月板股骨韧带对外侧半月板外突的影响。方法:本研究为回顾性病例对照研究。2013年1月至2015年1月于本院进行关节镜手术的病例中,选取存在外侧半月板后根部损伤的病例作为研究对象。通过MRI评估和关节镜资料确定入选病例的损伤类型,并按改良的Peterson分型分为MFL完整组和MFL缺失组。选取同期进行前交叉韧带重建手术而外侧半月板正常的病例作为对照组。测量各组半月板外突值,并统计各组外突发生率和外突值的差异。结果:共有72例病例入选本研究,其中MFL完整型56例,外突发生率为30.4%,外突均值为0.44±0.11 mm;MFL缺失型为16例,外突发生率为100%,外突均值为1.95±0.21 mm;匹配对照组56例,外突发生率为1.8%,外突均值为0.40±0.20 mm。MFL完整组和对照组的外突均值无显著差异(P=0.823),MFL缺失组的外突均值要显著大于MFL完整组(P<0.001)和对照组(P<0.001)。结论:半月板股骨韧带对限制外侧半月板外突有重要作用。外侧半月板后根部损伤时,MFL完整型并不出现明显的半月板外突,而MFL缺失型则会出现明显的半月板外突。
Objective To examine the influence of meniscofemoral ligament(MFL) on the lateral meniscus extrusion. Methods 72 patients with posterior lateral meniscus root lesion(PLMRL) received arthroscopic surgery in our department between January 2013 and January 2015. The outcome of the surgery was evaluated in this study retrospectively. The injury patterns were verified by MRI and arthroscopic images. According to modified Peterson classification system,the patients were divided into MFL intact group(n =65) and MFL absent group(n =16). 56 gender- and BMI-matched patients with ACL injury and normal lateral meniscus were recruited in the normal control group and they underwent ACL reconstruction at the same period.Extrusion rate and extrusion level were measured and compared among the three groups. Results The extrusion incidence was 30.4% in MFL intact group,100% in MFL absent group,and 1.8% in normal control group. The mean extrusion levels in MFL intact group,MFL absent group,and normal control group were 0.40± 0.20 mm,0.44 ± 0.11 mm and 1.95 ± 0.21 mm,respectively. The mean extrusion level of the MFL absent group was significantly greater than that in the other two groups(P〈0.001). Conclusions MFL serves as an important structure for resisting lateral meniscus extrusion.
