ADC直方图区分宫颈癌常见病理类型的价值 被引量:29

The Value of Histogram-based Apparent Diffusion Coefficient in Distinguishing Common Pathological Subtypes of Cervical Cancer

摘要 目的:探讨磁共振弥散加权成像(DWI)的表观弥散系数(ADC)直方图对两种常见宫颈癌病理类型的鉴别诊断价值。方法:回顾性分析经病理证实的IB-IIA期宫颈癌37例。在磁共振工作站上绘制整个肿瘤病灶的ADC直方图,记录肿瘤各百分比ADC值及ADCmin、ADCmean、ADCmax;并利用Fire Voxel软件计算出肿瘤ADC直方图曲线的偏度和峰度,分别对鳞癌和腺癌进行统计学比较。结果:鳞癌的ADCmin明显低于腺癌(P=0.042),其余各百分比ADC值、ADCmean、ADCmax在两组间没有统计学差异。鳞癌和腺癌的ADC直方图均为正偏态分布,鳞癌的偏度及峰度均较腺癌高(P=0.027和0.017)。结论:ADC直方图可以客观地提供肿瘤的整体信息,能区别鳞癌和腺癌之间的差异,对宫颈癌的诊断有潜在优势。 Purpose: To evaluate whether histogram analysis of apparent diffusion coefficient (ADC) values can be used to differentiate two common pathological subtype of cervical cancers. Methods: Thirty-seven cervical cancer patients with FIGO stage IB-IIA were retrospectively analyzed. ADC histogram was obtained from the entire tumor volume. ADC percentile values, ADCmin, ADC ADCma~ were obtained in a MR workstation. The skew and kurtosis of the histogram were calculated through FireVoxel software. Results: The ADCmin was significantly lower in squamous cell carcinomas. The distribution of ADC values was positively skewed in both groups. Histogram skew and kurtosis of squamous cell carcinoma were significantly higher than that of adenocarcinoma. Conclusion: Histogram analysis of ADC values in cervical tumors has potential for differentiating histologic subtypes, providing information about the entire tumor.
