新西兰雄兔膀胱输尿管连接部损伤与膀胱输尿管返流的相关性研究

The correlation between ureterovesical junction injury of New Zealand male rabbits and vesicoureteral reflux

摘要 目的探究新西兰雄兔膀胱输尿管连接部损伤与膀胱输尿管返流的相关性及意义。方法将40只新西兰雄兔随机分为空白对照组8只,假手术组8只,手术组A8只,手术组B8只,手术组C8只,手术组分别破坏兔膀胱输尿管连接部长度的1/3、2/3、全部,在膀胱内注入造影剂后观察是否发生膀胱输尿管返流及其程度,并进一步探究各组膀胱容量、同侧肾盂内压力、肾脏组织的病理改变。结果对照组、假手术组、手术组A、手术组B均未出现膀胱输尿管返流,手术组C均出现不同程度返流,其中I级1例、II级2例、III级5例,并且此组膀胱容量显著减少、肾盂内压力明显升高,肾脏病理检查肾盂扩张并伴有移行细胞上皮层扁平化,部分肾脏(4/8例)可以观察到肾小管上皮细胞局灶性变薄及肾间质增宽。结论膀胱输尿管连接部与膀胱输尿管返流有直接相关性;破坏膀胱输尿管连接部2/3以内的长度不会造成膀胱输尿管返流;在发生膀胱输尿管返流后肾盂内压力升高,长时间持续的升高可能造成肾功能的损害。 Objectives To explore the correlation and significance between ureterevesical junction injury of New Zealand male rabbits and vesicoureteral reflux. Methods 40 New Zealand rabbits were randomly divided into control group, sham operation group, surgery group A, surgery group B, surgery group C, destroyed the length of uret- erovesical junctionl of 1/3,2/3 ,3/3. To observe whether the occurrence of VUR and degree, and to measure bladder capacity and the pressure in the renal pelvis and pathological changes in kidney tissue. Results In control group, sham operation group, surgery group A, surgery group B, had no VUR, surgery group C had different degrees of reflux, which contained level I lcase, II Level 2 cases, III 5 cases, and in this group there were significantly reduced bladder capacity and increased the pressure in the renal pelvis, renal pelvis dilation with thinning of the transitional cell epithelial layer was observed, focal thinning of the tubular epithelium and interstitial widening were observed in part of the kidney(4/8 cases). Conclusions Ureterovesical junction and vesicoureteral reflux are directly corre- lated;Destruction of Ureterovesical junction within two - thirds of the length does not cause vesicoureteral reflux;Pelvis within the pressure increases when vesicoureteral reflux happens, the long - lasting increase may cause damage to kidney function.
