摘要
目的观察替米沙坦对高血压患者血管功能和动脉粥样斑块的影响。方法选取医院收治的高血压患者80例,随机分为对照组和治疗组,各40例。对照组患者给予酒石酸美托洛尔片25 mg口服治疗,每日1次,治疗组患者给予替米沙坦片80 mg口服治疗,每日1次。2周为1个疗程,两组患者均治疗4个疗程。检测并记录两组患者治疗前后颈动脉内-中膜厚度(IMT)、颈动脉斑块大小及颈动脉血流动力学参数水平变化,观察两组患者的疗效及不良反应发生情况。结果与治疗前相比,两组患者的颈动脉IMT、斑块大小和血流动力学参数、各项血液指标水平均明显降低,且治疗组下降更明显(P<0.05);治疗总有效率总有效率为87.50%,明显高于对照组的80.00%(P<0.05);两组均未见明显药品不良反应。结论替米沙坦在平稳降压的同时,可延缓甚至逆转颈动脉IMT的进展,降低动脉粥样斑块的发展,改善动脉血流动力学水平,值得临床推广。
Objective To observe the influence of telmisartan on the vascular function and atherosclerotic plaque in the patients with hypertension. Methods Totally 80 patients with hypertension admitted to our hospital were selected and randomly divided into the treatment group and the control group,40 cases in each group. The control group was treated with oral Metoprolol Tartrate Tablet 25 mg,once daily,while the treatment group was treated with Telmisartan Tablet 80 mg,once daily. Two weeks were taken as 1 course of treatment and 4 courses were given to the cases in each group. The changes of carotid arterial intima- media thickness( IMT),arterial plaque size and arterial hemodynamic parameter levels were detected and recorded before and after treatment. The treatment efficacy and occurrence situation of adverse reactions during treatment were observed. Results Compared with before treatment,IMT,arterial plaque size,arterial hemodynamic parameters and various homological indicators levels in the two groups were significantly decreased,moreover the decrease in the treatment group was more significant( P〈0. 05). The total effective rate in the treatment group was 87. 50%,which was signficantly higher than 80. 00% in the control groups( P〉0. 05); no obvious adverse reactions were found in the two groups. Conclusion Telmisartan can decrease blood pressure smoothly,at the same time can delay or even reverse the progression of carotid arterial IMT,reduces the arterial plaque development,improves the arterial hemodynamic level and is worthy of clinical promotion.
