上颌快速扩弓对颅颌面骨及上气道影响的锥形束CT分析 被引量:8

摘要 目的 研究上颌快速扩弓(rapid maxillary expansion,RME)后颌骨宽度、牙弓宽度和鼻咽、口咽容积的三维变化,为正畸临床提供参考.方法 选择35例上颌骨横向发育不足的患者,其中男性18例,女性17例,平均(12.1±1.1)岁.所有患者接受RME治疗.扩弓前、扩弓结束时(扩弓16d)及扩弓后3个月拍摄锥形束CT并进行三维重建和测量.结果 扩弓结束时鼻外侧壁宽度[(23.96±2.68) mm]和上颌骨宽度[(60.86±3.64) mm]分别比扩弓前[分别为(22.23±2.48)和(57.65±3.58) mm]增加(1.73±0.20)和(3.21±0.06) mm,扩弓前后差异均有统计学意义(P<0.05);扩弓后3个月上颌骨宽度[(59.79±2.98) mm]与扩弓结束时相比差异无统计学意义,提示上颌骨宽度扩弓效果较稳定.扩弓结束时鼻咽段气道容积[(4 496±983)mm3]与扩弓前[(3 462±1 147) mm3]相比增加29.9%,扩弓前后差异有统计学意义(P<0.05),扩弓后3个月鼻咽段容积保持相对稳定.结论 RME可显著增加鼻上颌骨复合体的宽度和鼻咽段气道容积. Objectives To evaluate the naso-maxillary complex width and pharyngeal airway volume changes after rapid maxillary expansion(RME).Methods Thirty-five patients were selected(18 males,17 females,mean age,12.1 ± 1.1 years).All patients underwent orthodontic treatment with Hyrax palatal expanders.Cone-beam CT(CBCT) scan was taken before treatment(T0),16 days(T1) and three months (T3) after RME.Naso-maxillary complex width and pharyngeal airway volume were measured.Results After treatment the width of piriform aperture and maxillary width were significantly increased compared with that before treatment(P〈0.05).Three months after RME,no statistical difference was found in maxillary width compared with that before treatment.The nasopharyngeal volume significantly increased by 29.9% compared with that before treatment(P〈0.05),and the volume remained relatively stable after three months.Conclusions RME resulted in a significant increase in the naso-maxillary complex width and nasopharyngeal volume.
