摘要
目的通过分析CK10,Ki-67和Bcl-2抗原蛋白在日光性角化病和基底细胞癌中的不同表达,探讨二者在细胞分化、增殖和凋亡等生物学行为方面的差异,深入理解两种疾病以及对疾病的治疗和预后提供理论指导。方法用免疫组化的方法观察39例AK和40例BCC中CK10,Ki-67和Bcl-2蛋白的表达,并对结果进行统计学分析。结果 CK10在AK和BCC的表达差异有统计学意义(P<0.001),前者阳性表达远高于后者;Ki-67在AK中阳性表达率明显低于BCC,二者表达差异有统计学意义(P<0.001);Bcl-2在AK中阳性表达率尽管较高(79.49%),但低于BCC(92.50%),两者相比差异有统计学意义(P<0.001)。结论研究进一步证明尽管日光性角化病可以向鳞状细胞癌进化,但是很少发展成侵袭性鳞癌;而基底细胞癌癌细胞保持未分化,一般不转化成鳞状细胞癌,虽然侵袭性较日光性角化病强,但不易发生转移的临床生物学行为。
Objective To explore the differences between actinic keratosis (AK) and basal cell carcinoma (BCC) in cell differentiation, proliferation and apoptosis and other biological behavior, we analyzed the different expres- sion of a variety of antigenic proteins in AK and BCC, providing theoretical guidance for the treatment and prognosis of both diseases. Methods Immunohistochemical techniques were used to detect the expression of CK10, Ki-67 and Bcl-2 protein in 39 cases of AKs and 40 cases of BCCs, and the results were statistically an- alyzed. Results The expression of CK10 showed significantly difference between AKs and BCCs (P 〈 0. 001 ) ,and the positive expression in AKs is much higher than in BCCs. The positive expression rate of Ki- 67 in AKs was apparently lower than in BCCs, and there was significantly difference between two groups (P 〈 0. 001 ). Despite that the positive expression rate of Bcl-2 in AKs was high (79.49%), but lower than in BCCs (92. 50% ), and there was significantly difference ( P 〈 0. 001 ). Conclusions This study suggests that actinic keratosis can develop into squamous cell carcinoma, but rarely into invasive squamous carcinoma (SC). BCC cells maintain undifferentiated and usually don't transfer into SC. Although BCCs are more ag- gressive than AKs, it is less prone to metastasis.
