摘要
目的采用苯磺酸左旋氨氯地平(施慧达)治疗高血压合并2型糖尿病患者,观察其血压、血清25-羟维生素D(25(OH)D)及肾素水平的变化以及三者间的相关性。方法选取新诊断高血压合并2型糖尿病患者40例,给予施慧达降压治疗,维持口服降糖药或胰岛素控糖方案。比较治疗前后患者血清25(OH)D、肾素及血压的变化,分析血清25(OH)D与肾素及血压的相关性。结果与治疗前比较,治疗后患者血清25(OH)D水平显著升高[(28.6±3.3)μg·L-1比(15.5±5.0)μg·L-1,P<0.05];血压及肾素水平显著降低[(124.5±21.6)/(81.5±9.5)mmHg比(165.2±28.0)/(102.1±15.6)mmHg,(2.53±0.65)ng·mL-1·h-1比(3.21±0.71)ng·mL-1·h-1,均P<0.05];血清25(OH)D水平与收缩压、舒张压、肾素呈显著负相关(r=-0.45,r=-0.38,r=-0.48,均P<0.05)。结论采用施慧达治疗高血压并2型糖尿病患者,可降低血压及肾素水平,提高血清25(OH)D水平,血清25(OH)D水平与血压、肾素水平分别呈负相关关系,从而可能减少糖尿病高血压病变的风险。
Objective To observe the changes in blood pressure,serum 25-hydroxyvitamin D (25(OH)D)levels and rennin levels after treatment with levamlodipine besylate tablets and their correlations in hypertensive patients with type 2 diabetes mellitus.Methods Forty patients with new diagnostic hypertension and type 2 diabetes mellitus were treated with levamlodipine besylate tablets and oral hypoglycemic agents or insulin therapy.Serum 25(OH)D and rennin levels and blood pressure were measured before and after treatment.The relationships of 25(OH)D to ren-nin and blood pressure were analyzed.Results Serum 25(OH)D levels measured after treatment were significantly higher than those measured before treatment[(28.6±3.3)μg·L-1 vs (15.5± 5.0)μg·L-1 ,P <0.05].Blood pressure and rennin levels measured after treatment were signifi-cantly lower than those measured before treatment[(124.5 ± 21.6)/(81.5 ± 9.5 )mmHg vs (165.2±28.0)/(102.1±15.6)mmHg and (2.53±0.65)ng·mL-1 ·h-1 vs (3.21±0.71)ng· mL-1 ·h-1 ,respectively;P <0.05)].Serum 25(OH)D levels were negatively correlated with sys-tolic pressure,diastolic pressure and rennin levels(r=-0.45,r=-0.38 and r=-0.48,respectively;P <0.05).Conclusion Levamlodipine besylate tablets can lower blood pressure,decreaserennin levels and increase 25(OH)D levels in hypertensive patients with type 2 diabetes mellitus.Serum 25(OH)D levels are negatively correlated with blood pressure and rennin levels.Therefore,levamlodipine besylate tablets may reduce the cardiovascular risk in hypertensive patientswith type 2 diabetes mellitus.
