摘要
目的：研究急性脑出血与多器官功能衰竭的相关临床因素及防治措施。方法：收集2012年5月至2015年3月我院收治的急性脑出血患者病例200例，其中并发多器官功能衰竭者60例。分析患者临床特点，包括多器官功能障碍综合征（MOF）发生率、发生时间、基础疾病、MOF器官数与死亡率。结果：患者继发MOF发生于术后4～10d，合并的基础疾病主要有高血压病、肺心痛、心脏病、脑卒中史、糖尿病、高甘油三酯血症、高胆固醇症以及消化性疾病，以高血压病最为多见；在衰竭器官中，受累频度主要以肺（70．00％）；胃肠（55．00％）；心脏（41．67％）多见；60例继发MOF的患者中，死亡44例，占7333％；其中继发衰竭器官为2个者19例，死亡8例，占42．11％；衰竭器官3个者26例，死亡21例，占80．77％；器官功能衰竭／〉4个者15例，死亡率100％，衰竭器官数越多，急性脑出血死亡率越高。结论：急性脑出血并发多器官功能衰竭病发和死亡率高，准确分析、评价器官功能对于降低死亡率具有重要意义。
Objective： To research the clinical relative factors and prevention measures and mul tiple organ failure on acute cerebral hemorrhage. Methods ： The research col- lected 200 patients from 2012 May to 2015 acute cerebral in March in our hospital with hemorrhage among which 60 cases were complicated with multiple organ failure and analyze clini- cal characteristics of these patients, including multiple organ dysfunction syndrome （MOF） incidence, occurrence time, und erlying diseases, MOF the number of organs and mortality. Results ： The patients with secon dary MOF occurred in the postoperative 4 - 10 D, mainly include hypertension, pulmonary heart disease, heart disease, stroke, diabetes history, hy- pertriglycefidemia, h yper cholesterolemia and di gestive diseases foundation combined with hyper tension disease, see more most; in failure organs, suffer tired frequency mainly in lung （70%） ; gastrointestinal （55%） ; hea rt （41.67%） see; 60 patients with secondary MOF, 44 cases of death, a ccounting for 73.33% ; 19cases of 2 in which secondary organ failure, 8 cases of death, accounting for 42.11% ; 26 cases of organ failure in 3, 21 cases death, accounting for 80.77% ; the 4 organ failure including 15 cases, and mortality wasl00%. The more the number of failure organs, the higher the mortality rate of acute cerebral hemorrhage. Conclusion ： The acute cerebral hemorrhage c omplicated with multiple organ failure disease and high mortality, the accurate analysis, evaluation o f organ function has an important significance for reducing mortality.
