妊娠妇女尿碘含量与妊娠期甲状腺功能降低的相关性 被引量:12

摘要 目的:探讨妊娠妇女尿碘含量与妊娠期甲状腺功能降低的关系。方法:将产检的妊娠妇女346例作为研究对象,依据孕周分为早期妊娠组、中期妊娠组和晚期妊娠组;采集孕妇空腹晨尿及静脉血样,检查晨尿样本中碘含量测定静脉血标本中促甲状腺素(TSH)、游离三碘甲状腺原氨酸(FT3)和游离甲状腺激素(FT4)等指标,分析妊娠妇女尿碘含量与妊娠期甲状腺功能降低关系。结果:346例共检出甲状腺功能降低47例(13.58%),其中尿碘水平降低患者42例(12.14%),尿碘水平适量、超足量5例(1.45%),两者比较差异有统计学意义(P<0.05);尿碘中位数(MUI)>250μg/L孕妇的FT3及HCG指标明显低于碘适量孕妇(P<0.025),FT4及TSH比较无统计学意义;而MUI<150μg/L,孕妇FT4水平显著高于碘适量孕妇(P<0.05),FT3、TSH、HCG比较无统计学意义;妊娠期各组孕妇FT3水平显著高于正常参考值,而TSH水平低于正常参考值,早中晚期FT4均显著低于正常参考值,同时早中晚妊娠期血清FT4水平差异具有统计学意义,在血清标本检测中发现,妊娠期各组孕妇FT3水平明显高于参考值,差异均有统计学意义(P<0.05);早中晚妊娠期血清FT4水平差异具有统计学意义(P<0.05)。结论:妊娠期妇女尿碘水平可反映甲状腺功能,尿碘水平低于正常,甲状腺功能减退几率增加。 Objective:To investigate the relationship between urine iodine content in pregnant women and thyroid dysfunction. Methods:A total of 346 cases on the basis of pregnancy were divided into early pregnancy group,mid pregnancy group,late pregnancy group. Fasting morning urine and venous blood samples were collected,TSH of venous blood samples,FT3 and FT4 were tested. Re-sults:A total of 346 cases of pregnant women detected 47 cases with thyroid dysfunction(13. 58%), with which 42 cases showed urine iodine level decrease(12. 14%),5 cases showed urine iodine level appropriate and adequate(1. 45%),the difference had statistical significance(P﹤0. 05);FT3 and hCG indexes of pregnant women with MUI≥250 μg/L were significantly lower than the number of pregnant women with appropriate iodine( P ﹤0 . 025 ),FT4 and TSH showed no statistical signifi-cance;MUI≤150 μg/L,FT4 levels in pregnant women were significantly higher than that in the io-dine suitable pregnant women(P﹤0. 05),FT3,and TSH,hCG comparison results showed no statisti-cal significance. The level of FT3 pregnancy groups was significantly higher than that of normal refer-ence value,and TSH levels were lower than the normal reference value;early,middle and late preg-nancy FT4 were significantly lower than the normal reference value,and early,middle and late preg-nancy serum FT4 level difference showed statistical significance. In the serum samples,FT3 levels of pregnancy women were significantly higher than the reference value,differences were statistically sig-nificant(P﹤0. 05);early,middle and late pregnancy serum FT4 level differences were statistically significant(P﹤0. 05). Conclusion:Pregnancy urine iodine content can reflect thyroid function,MUI level is lower than normal reference value,thyroid dysfunction odds fluctuate.
