摘要
目的总结Stanford B型主动脉穿透性粥样硬化性溃疡腔内治疗的手术时机及治疗效果。方法回顾分析我院2009年1月至2014年6月共32例StanfordB型主动脉穿透性粥样硬化性溃疡临床资料，其中单纯性主动脉穿透性粥样硬化性溃疡4例，合并主动脉壁内血肿患者28例。所有患者均采用主动脉腔内覆膜支架术。结果除1例（3％）患者术后3个月死亡，1例（3％）患者支架远端出现假性动脉瘤二次行支架植入术外，其余患者术后1、3、6、12个月及之后每年均行CTA复查，效果好。结论对于主动脉穿透性粥样硬化性溃疡患者，应采取积极的治疗方案。单发型主动脉穿透性粥样硬化性溃疡早期采取覆膜支架腔内修复术效果佳；伴发主动脉内壁间血肿的主动脉穿透性粥样硬化性溃疡患者应控制血压、心率，治疗2周，复查CTA血肿吸收后，再行覆膜支架腔内修复术效果更佳。
Objective To summarize the surgical opportunity and effect of endovascular treatment for B Stanford aortic penetrating atherosclerotic ulcer. Methods Retrospective analysis of our hospital between January 2009 and June 2014, a total of 32 cases of Stanford type B aortic penetrating atherosclerotic ulcer clinical data, including simple aortic penetrating atherosclerotic ulcer in 4 cases, combined with aortic intramural hematoma 28 cases, all patients were treated with aortic lumen coated stent. Results Except for 1 case （3%） patients with postoperative death after 3 months, 1 ease （3%） with distal stent pseudoaneurysms and received secondary stent implantation, all of the patients after 1, 3, 6, 12 months and after each line of CTA review effect were good. Conclusion For patients with aortic penetrating atheroselerotie ulcer, should take active treatment, early adopting coated stent showed better effects for a simple aortic penetrating atheroselerotie ulcer. With aortic wall hematoma in aortic penetrating atheroselerotie ulcer patients should control blood pressure and heart rate, and after treatment for 2 weeks, the effect of stent graft is better after review the CTA hematoma absorption.
