摘要
目的:探究盐酸戊乙奎醚在上颌骨多发性骨折手术中的麻醉效果及适宜剂量。方法:选择2013年9月-2014年9月在本院行上颌骨多发性骨折手术的患者60例作为研究对象,采用随机数字表法分为三组,每组20例。于术前30 min时给予A组肌注阿托品0.01 mg/kg、B组肌注盐酸戊乙奎醚0.01 mg/kg、C组肌注盐酸戊乙奎醚0.015 mg/kg,均行表面麻醉及全身麻醉。记录给药前,给药后15、30 min,插管后2 min及拔管后2 min时,患者的BP、T、HR、口腔分泌物等指标。结果:A组用药后心率、血压及体温均明显增加,与用药前及B、C组比较差异均有统计学意义(P<0.05);三组用药后口腔分泌量均明显减少,与用药前比较差异有统计学意义(P<0.05);A组在拔管后口腔分泌量较B、C组明显增加,比较差异有统计学意义(P<0.05)。结论:盐酸戊乙奎醚抑制腺体分泌时间较阿托品长,且对心率、血压及体温无明显的影响,更适用于上颌多发骨折手术的术前用药,低剂量盐酸戊乙奎醚能达到高剂量组相同的术前麻醉效果。
Objective :To investigate the anesthetic effect of Penehyclidine Hydrochloride on the preoperative anesthesia of maxillary multiple fractures and explore its suitable dosage.Method: 60 patients with operations of maxillary multiple fractures in our hospital from September 2013 to September 2014 were selected and randomly divided into three groups, with 20 cases in each group.30 minutes before the operation, group A were given intramuscular injection of Atropine 0.01 mg/kg, group B were given intramuscular injection of Penehyelidine Hydrochloride 0.01 mg/kg, group C were given intramuseular injection of Penehyelidine Hydrochloride 0.015 mg/kg.They were all given surface anesthesia and general anesthesia.The indexes of BP, T, HR and oral secretions were recorded before drug administration, 15 and 30 minutes after drug administration, 2 minutes after intubation and 2 minutes after extubation.Result: The heart rate, blood pressure and temperature of group A increased signifieantly, eompared with before drug administration and group B, C the difference was statistically significant ( P〈0.05 ) .Oral cavity seeretion volume of the three groups reduced significantly than before drug administration, the differenee was statistically significant ( P〈0.05 ) .2 minutes after extubation, oral cavity secretion volume of group A increased more significantly than group B and group C, the differences were statistieally significant (P〈0.05) .Conclusion: Compared with Atropine, Penehyclidine Hydrochloride can inhibit glandular secretion much longer and it has no obvious influence on blood pressure, heart rate and body temperature, it is more suitable for the operation of maxillary ultiple fractures.Low dose of Penehyclidine Hydrochloride can achieve the same preoperative anesthetic effeet with the high dose.
