摘要
目的观察抗胸腺球蛋白(ATG)联合环孢素A(Cs A)治疗儿童再生障碍性贫血(AA)的疗效,探讨提高儿童AA疗效的有效方案。方法回顾性分析52例在我院接受正规治疗并且门诊随访时间超过6个月AA患儿的临床资料,其中重型再障(SAA)24例,慢性再障(CAA)28例,所有患儿按治疗方案不同分为2组,对照组为单用Cs A治疗,治疗组为ATG联合Cs A治疗,比例两组患儿的疗效。结果 CAA组患儿的总有效率为82.1%(23/28),SAA患者总有效率为50.0%(12/24);28例CAA患儿中接受对照组治疗27例,总有效率为81.5%(22/27),接受治疗组治疗1例,总有效率100.0(1/1),两组有效率比较,差异无统计学意义(P>0.05);24例SAA患儿中接受治疗组治疗8例,总有效率25.0%(2/8),接受对照组治疗16例,总有效率75.0%(12/16),治疗组有效率高于对照组,差异具有统计学意义(P<0.05)。结论 ATG联合Cs A治疗儿童SAA疗效佳,可作为缺乏合适供体进行造血干细胞移植患儿的首选治疗方式。
Objective The purpose of the study is to observe the effect of the anti-thymocyte globulinlobulin(ATG)combined with cyclosporine(CsA)on the treatment of children with aplastic anemia(AA).Methods 52 cases with AA in our hospital were analyzed and the patients were folowed up for more than 6 months.Severe aplastic anemia(SAA)in 24 cases and chronic aplastic anemia(CAA)in 28 cases were included.Al the patients were divided into two groups.The experiment group was treated with ATG and CsA.Control group was treated using CsA.Results The total effective rate in CAA patients was 82.1%(23/28).The total effective rate in SAA was 50%(12/24).Effect of CAA group was significantly higher than the SAA group.In the 28 patients with CAA,27 cases were treated in the control group,the total effective rate of which was 81.5%(22/27).One case was in the treatment group,and total effective rate was 100.0%(1/1).The difference between the two groups was not statisticaly significant(P &gt; 0.05).In the 24 patients with SAA,8 cases were in the experiment group and the total effective rate was 25%(2/8).16 cases were in the control group,and the total effective rate 75%(12/16). The effective rate in the treatment group was higher than the control group(P 〈 0.05).Conclusion ATG combined with CsA is effective in children with SAA.The method could be preferred for the children without suitable donors for hematopoietic stem cel transplantation.
