摘要
目的 探讨采用掌侧锁定钢板内固定治疗不稳定桡骨远端骨折的疗效。方法 自2010-03—2012-03诊治不稳定桡骨远端骨折35例,采用切开复位,掌侧锁定钢板内固定。结果 除1例失访外,其余患者获得平均30.2个月（24~48个月）的随访。X线片显示骨折全部一期愈合,平均愈合时间为8周。1例骨缺损严重及5例骨质疏松患者,术中植入人工骨。患者均无感染、骨不连、钢板松动、腕管综合征等并发症发生。疗效按Cooney腕关节评分标准评定：优19例,良11例,可3例,差1例,优良率88.2%。结论 采用掌侧锁定钢板内固定治疗不稳定桡骨远端骨折,疗效确切,可以提供坚强内固定,早期功能锻炼,恢复正常的掌倾角、尺偏角、桡骨远端长度,腕关节Cooney评分满意。
Objective To observe the clinical outcome of volar locking plate for unstable distal radius fractures. Methods From March 2010 to March 2012,35 patients with unstable distal radius fractures were surgically treated with volar locking plate. Results Thirty-four patients were followed up for an average of 30.2 （range, 24-48） months. The X-ray films showed primary union in all patients in a mean healing time of 8 weeks. Artificial bones were implanted in 6 eases. No non-union, loosening of nails or earpal tunnel syndrome, infeeflon was found. At final functional assessment, the scores of 19 patients were excellent, 11 good, 3 fair and 1 poor aceordifig to Cooney＇s Clinical Scoring Chart. The excellent and good rate was 88.2%. Conclusion The volar locking plate system can offer a rigid fixation, early exercise for unstable distal radius fractures, leading to satisfaetory volar flit, radial inclination, and ulnar variance, with excellent clinical outcome.
