摘要
目的探讨妊娠期高血压病(PIH)患者血清和胎盘组织中血管细胞黏附分子-1(VCAM-1)、妊娠相关血浆蛋白-A(PAPP-A)表达情况及相关性。方法选取118例PIH患者作为观察组和40健康孕妇作为对照组,采用酶联免疫吸附法测定两组孕妇血清VCAM-1、PAPP-A水平,免疫组化法检测两组孕妇胎盘组织中VCAM-1、PAPP-A表达情况。分析两者的相关性。结果观察组血清VCAM-1、PAPP-A水平高于对照组(t=129.545、26.903,均P=0.000),妊娠高血压组、轻度子痫前期组、重度子痫前期组血清VCAM-1、PAPP-A水平逐渐升高,组间比较(F=6.826、4.713,P=0.001、0.024);对照组胎盘组织中VCAM-1阳性率及染色评分均高于观察组(χ2=66.043、83.028,t=16.670、27.510,均P=0.000);观察组胎盘组织中PAPP-A阳性率及染色评分均高于观察组(χ2=83.300、63.974,t=26.062,P值均为0.000);PIH患者血清VCAM-1和PAPP-A水平呈正相关(r=4.681,P=0.018),胎盘组织中VCAM-1和PAPP-A阳性表达情况呈负相关(r=-3.219,P=0.026)。结论 VCAM-1和PAPP-A共同参与了PIH的病理过程,是PIH重要的发病机制之一。
【Objective】To investigate the expression and correlation of vascular cell adhesion molecules-1(VCAM-1) and pregnancy associated plasma protein-A(PAPP-A) in serum and placental tissue in patients with pregnancy induced hypertension(PIH). 【Methods】118 patients with PIH were chosen as the observation group and 40 healthy pregnant women were chosen as the control group, the serum VCAM-1, PAPP-A levels were detected by enzyme linkedimmunosorbent assay( ELISA),the expression of VCAM-1, PAPP-A was detected by immunohistochemistry in placenta tissue. And analysed the correlation between them. 【Results】The serum levels of VCAM-1, PAPP-A were higher than those of the control group(t=129.545, 26.903, P=0.000), the serum levels of VCAM-1, PAPP-A of gestational hypertension group, mild preeclampsia group and severe preeclampsia group increased gradually, compared between the groups(F=6.826, 4.713, P=0.001, 0.024); The positive rate of VCAM-1 and dyeing scores in placental tissues of the control group were significantly higher than those of the observation group(χ2=66.043, 83.028, t=16.670, 27.510, P=0); The positive rate of PAPP-A and staining scores in placenta were higher than those of the observation group(χ2=83.300, 63.974, t=26.062, P= 0.000); The serum levels of VCAM-1 and PAPP-A of patients with PIH were positively correlated(r=4.681, P=0.018), the positive expression of VCAM-1 and PAPP-A in placenta tissue was negatively correlated(r=-3.219, P=0.026). 【Conclusion】 VCAM-1 and PAPP-A participate in the pathological process of PIH, it is one of the most important mechanisms of PIH.
