名中医支气管扩张医案数据关联性分析 被引量:9

摘要 目的:探索数据分析技术在中医医案分析中的作用及支气管扩张中医诊疗规律。方法:采用数据分析方法分析名中医治疗支气管扩张医案的四诊信息,以及中药之间的关联关系。结果:症状频次显示,咯痰、咳嗽、咯血、喘息、胸闷、食欲差、口干、发热、乏力、便秘、自汗、睡眠差为主要症状,舌质红苔黄和黄腻、细脉为主要舌脉表现。因子分析显示,痰热蕴肺证和气阴两虚证为支气管扩张的主要证候。药物频次分析显示,麦冬、黄芩、甘草、鱼腥草、紫菀、桔梗、生地黄、当归、桑白皮等为主要药物。因子分析显示,百合固金汤、桔梗汤、麻杏石甘汤、千金苇茎汤、苏子降气汤等为主要方剂。关联分析显示,桔梗、黄芩、鱼腥草;鱼腥草、桑白皮、黄芩;麦冬、桔梗、鱼腥草;鱼腥草、北沙参、麦冬;生地黄、百合、麦冬;桑白皮、麦冬、黄芩等药对在医案中经常联合使用。结论:基本证候分布规律、药物配伍规律也与支气管扩张的临床认识和治疗相符合,说明支气管扩张的辨证施治,具有较为显著的规律性,数据挖掘技术方法能够适用于中医医案研究。 Objective: To explore the function of data analysis technique in the analysis of TCM medical cases and the diagnostic and treatment law of bronchiectasis. Methods: Data analysis method was adopted to analyze the data of four diagnostic techniques of famous medical cases of bronchiectasis,as well as the correlation between the Chinese herbs. Results: According to the frequency manifestations,the main symptoms included coughing up phlegm,cough,coughing up blood,gasp,chest distress,poor appetite,dry mouth,fever,lassitude,constipation,spontaneous sweating,poor sleep quality,red tongue with yellowish and greasy tongue coating,thin pulse. The factors analysis showed that the syndrome of phlem-heat accumulating the lung and syndrome of both qi and yin deficiency are the chief syndrome of bronchiectasis. The analysis of drug use frequency showed that Maidong( Radix Ophiopogonis),Huangqin( Radix Scutellariae),Gancao( Radix et Rhizoma Glycyrrhizae),Yuxingcao( Herba Houttuyniae),Ziwan( Radix et Rhizoma Asteris),Jiegeng( Radix Platycodonis),Shengdihuang( Radix Rehmanniae),Danggui( Radix Angelicae Sinensis),Sangbaipi( Cortex Mori) were the primary herbs. The factor analysis showed that,Lily Bulb Metal-Securing Decoction,Platycodon Decoction,Ephedra,Apricot Kernel,Gypsum and Licorice Decoction,Phragmites Stem Decoction,Perilla Fruit Qi-Descending Decoction were the main formulas. The correlation analysis showed that the following combinations were often used in the medical cases: Jiegeng( Radix Platycodonis),Huangqin( Radix Scutellariae) and Yuxingcao( Herba Houttuyniae); Yuxingcao( Herba Houttuyniae),Sangbaipi( Cortex Mori),Huangqin( Radix Scutellariae); Maidong( Radix Ophiopogonis),Jiegeng( Radix Platycodonis) and Yuxingcao( Herba Houttuyniae); Yuxingcao( Herba Houttuyniae),Beishashen( Radix Glehniae) and Maidong(( Radix Ophiopogonis); Shengdihuang( Radix Rehmanniae),Baihe( Bulbus Lilii),and Maidong( Radix Ophiopogonis); Sangbaipi( Cortex Mori),Maidong( Radix Ophiopogonis) and Huangqin( Radix Scutellariae). Conclusion: The basis syndrome distribution law,drug compatibility regularity are in line with the clinical recognition and treatment of bronchiectasis. It indicates that the syndrome differentiation and treatment of bronchiectasis has relatively significant regularity; the data mining technique is applicable in the study of medical cases.
