摘要
目的:探讨腹腔镜治疗早期子宫内膜癌的近期临床疗效,以及对患者性功能的影响。方法:选择2012年7月至2014年7月符合标准的患者60例,随机分为观察组和对照组各30例,观察组采用腹腔镜手术,对照组采用传统手术,比较二者近期疗效和性功能变化情况。结果:观察组患者手术时间及术中清扫淋巴结个数与对照组相似,差异无统计学意义(P>0.05);观察组患者术中出血量、术后肛门排气时间、术后盆腹腔引流量、术后下床活动时间及术后住院时间均优于对照组,差异有统计学意义(P<0.05)。观察组患者并发症发生率6.67%,低于对照组的26.67%,差异有统计学意义(P<0.05)。术前两组患者国际女性性功能评估量表各项评分相似,差异无统计学意义(P>0.05);术后6个月观察组各项评分与本组术前相似,差异无统计学意义(P>0.05),但对照组各项评分均低于本组术前,差异有统计学意义(P<0.05)。结论:腹腔镜手术治疗早期子宫内膜癌近期疗效好、创伤小、恢复快,能有效保护患者的性功能,值得临床推广应用。
Objective： To investigate the short-term clinical effect of laparoscopic operation on early cancer of endometrium and the influence on sexual function of the patients. Method： A total of 60 eligible patients treated between July 2012 and July 2014 were selected, and randomly divided into observation group and control group, with 30 patients in each group. The observation group used laparoscopic operation, while the control group used traditional operation, and the short-telan effects and changes in sexual function were compared between the two groups. Result： The observation group had similar operative time and number of lymph nodes cleared in the operation to the control group, and the differences were statistically insignificant （P〉0.05） ; the observation group had better intraoperative blood loss, postoperative time to passage of gas by anus, postoperative drainage volume of abdominopelvic cavity, postoperative time to out-of-bed activity,and postoperative length of stay than the control group, and the differences were statistically significant （ all P 〈0.05 ）. The incidence of complications of the observation group was 6.67%, lower than that of the control group （26.67%）, and the difference was statistically significant （P〈0.05）. Before the operations, the pa- tients of the two groups had similar Brief Index of Sexual Function for Women （ BISF-W } scores, and the difference were statistically insignificant （P〉0.05 ） ; 6 months after the operation, the observation group had similar scores to those before the operation, and the differences were statistically insignificant （ all P〉0.05 ） ; but the control group had lower scores than those before the operation, and the differences were statistically significant （ all P〈0.05 ）. Conclusion ： Laparoscopic operation in treatment of early cancer of endometrium has good short-term effects, small wound, and rapid recovery, can effectively protect the sexual function of the patients, and consequently is worthy of clinical application.
