摘要
目的:观察活血化瘀法对髋部骨折患者围手术期D-二聚体的影响,探讨活血化瘀法对髋部骨折并发下肢深静脉栓塞的防治作用。方法:选我院治疗的髋部骨折患者46例,随机分为两组,每组各23例。治疗组术前1天、术后1周连续静滴0.9%NS250 m L+复方丹参注射液20 m L,每天1次。术后第1天开始服用桃红四物汤,每日1剂,连续服用1周;对照组术前1天、术后连续1周腹壁皮下注射低分子肝素(速碧林)0.4 m L,每天1次。两组均于术前1天、术后1、3、7天,清晨空腹抽血测定静脉血D-二聚体含量。结果:术前1天、术后1天、术后3天2组D-二聚体含量比较,差异无统计学意义(P>0.05),术后7天2组D-二聚体差异有统计学意义(P<0.05)。结论:活血化瘀法对髋部骨折患者围手术期血浆D-二聚体含量有降低作用,可有效降低髋部骨折术后下肢深静脉栓塞形成的风险,是一种安全、可行和有效的治疗方案。
Objective ： To observe the effect of blood-activating stasis-removing therapy on D-dimer of patients with hip fracture during perioperative period. Methods ： 46 cases of patients with hip fracture treated in our hospital were randomly divided into two groups,with 23 cases in each group. treatment group was given 0. 9% NS 250 mL intravenous drip continuously and compound danshen injections 20 mL once a day on the day before operation and in a week after operation,and took tao hong si wu tang once a day in a week after operation; Control Group was given abdominal subcutaneous injection of low molecular weight heparin（ Fraxiparine） 0. 4 mL once a day on the day before operation and in a week after operation. The D-dimer of fasting blood sample in the morning of two groups was measured 4 times（ i. e. the day before operation,the 1stday,the 3^rd day and the 7^th day after operation）. Results： Compared the content of D-dimer the day before operation,the 1^st day and the 3^rd day after operation between two groups,the difference had not statistical significance（ P〉0. 05）; The difference of the comparison between two groups on the 7^th day after operation was statistical significant（ P〈0. 05）. Conclusion：Blood-activating stasis-removing therapy can decrease D-dimer of patients with hip fracture during perioperative period and lower risks of leg deep-vein thrombosis after hip fracture operation. It is a safe,feasible and effective treatment protocols.
