摘要
目的：探讨新生儿坏死性小肠结肠炎的病理诊断和治疗方法。方法以我院2012年6月~2014年6月62例新生儿坏死性小肠结肠炎为研究对象，根据病理诊断资料，按治疗方法分为两组，在禁食、胃肠减压、平衡水电解质、营养支持等常规措施基础上，A 组32例选择抗感染治疗，B 组30例选择手术治疗。结果粪便隐血试验均呈阳性，镜检存在数量不等白细胞和红细胞，血培养细菌结果提示大肠埃希杆菌28例（45.16%），克雷白杆菌24例（38.71%），铜绿假单胞菌10例（16.13%）；A 组治疗总有效率87.5%，B 组治疗总有效率96.67%，组间比较无显著性差异（P ＞0.05）。结论病理检查是新生儿坏死性小肠结肠炎明确诊断的必要措施，根据病情选择治疗方案是提高治疗效果的关键。
Objective To study the pathological diagnosis and treatment methods of necrotizing enterocolitis in newborns. Methods 62 newborns with necrotizing enterocolitis who were treated in our hospital from June 2012 to June 2014 were selected as the research objects and were divided into two groups according to the pathological diagnosis data and treatment methods. Based on the conventional measures such as fasting, gastrointestinal decompression, equilibrium water-electrolyte and nutritional support and so on, 32 cases of A group were treated with anti-infection, while 30 cases of B group were treated with operation. Results Of the 62 cases, fecal occult blood test was positive, microscopic examination showed that there were different quantities of leukocyte and erythrocyte, the results of bacteria in blood culture showed that there were 28 cases escherichia coli（45.16%）, 24 cases klebsiella bacillus（38.71%）, and 10 cases pseudomonas aeruginosa（16.13%）. The total effective rate of treatment was 87.5% in A group and 96.67% in B group, there was no statistical significance between the two groups（P 〉 0.05）. Conclusion Pathological examination is necessary measure of the definite diagnosis on necrotizing enterocolitis in newborns, treatment method which was selected according to disease is the key to increase the treatment effect.
