摘要
目的:观察中药熏洗联合高锰酸钾溶液坐浴对混合痔PPH加外剥内扎术后患者创面疼痛的影响。方法:选取2013年2月—2015年2月本院收治的114例混合痔患者,随机分为对照组和治疗组各57例,两组患者均行PPH加外剥内扎术,对照组患者术后给予高锰酸钾溶液坐浴治疗;治疗组患者则术后加用中药经验汤剂熏洗治疗,两组均连续治疗1周。观察患者术后出血、创面肉芽生长、创面疼痛轻重程度变化情况。结果:对照组患者治疗后的创面愈合率52.6%较治疗组患者75.4%明显降低,差异具有统计学意义(P<0.05);治疗组患者术后出血、创面肉芽生长、创面疼痛等改善较对照组患者更为明显,差异具有统计学意义(P<0.05);治疗组患者不同治疗时间段的疼痛VAS评分较对照组患者更低,其创面愈合时间较对照组患者明显缩短,差异具有统计学意义(P<0.05)。结论:中药熏洗联合高锰酸钾溶液坐浴利于减少混合痔PPH加外剥内扎术后患者出血量,促进创面肉芽生长,缓解创面疼痛,加快创面愈合,疗效显著。
Objective： To observe the influence of TCM fumigation in combination with hip bath with potassiu,n permanganate sulutiun on the wound pain of patients with mixed hemorrhoids who have accepted PPH and external dissection and internal ligation. Methods： One hundred and fourteen patients with mixed hemorrhoids who were treated in our hospital between Feb. 2013 and Feb. 2015 were randomly divided into the control group and the treatment group, with 57 cases in each group. Both groups were given PPH plus external dissection and internal ligation. After the operation, the ones in the control group were given hip bath with potassium permanganate solution, while the ones in the treatment group were added TCM extverience decoction fumigation. Both groups were treated for one week continuously. Then bleeding, wound granulation and wound pain severity changes were ob- served ; The wound pain was assessed by using VAS ; The wound healing time was recorded and the healing rate of the wound was calculated. Results： After the treatment, the wound healing rate of the control group （ 52.6% ） was lower than that of the treatment group （75.4%）; And the difference between them had statistical significance（ P 〈0.05 ）. After the operation, the i,n- proved degrees of bleeding, wound granulation and wound pain severity of the treatment group were more obviously than those of the control group; And the differences between them had statistical significance （P 〈 0.05）. The pain VAS scores in different treatment periods of the treatment group were much lower than those of the control group; And its healing wound time was much shorter than that of the control group; And the differences between them had statistical significance （ P 〈 0.05 ）. Conclusion ： TCM fumigation in combination with hip bath with potassium permanganate solution has remarkable clinical curative effect on patieuts with mixed hemorrhoids who have accepted PPH and external dissection and intenal ligation, can reduce bleeding amount, promote wound granulation, release wound pain severity, and fasten wound healing.
