摘要
目的探讨3′-脱氧-3′-18 F-氟胸腺嘧啶胸苷(3′-deoxy-3′-18 F-fluoro-thymidine,18 F-FLT)分子显像中最大标准摄取值(maximum standardized uptake value,SUVmax)和病灶与对侧正常脑摄取比(L/N),在星形细胞瘤术后复发和放射治疗损伤中的定量分布特点,以利于准确地鉴别诊断。方法 2010-10-01-2013-12-31,对中山大学附属第一医院脑星形细胞瘤术后辅助放疗的29例患者,行18 F-FLT正电子发射及计算机断层显像(positron emission and computer tomography,PET-CT),利用感兴趣区(region of interest,ROI)技术进行定量分析,测定术后放疗病灶的SUVmax和L/N值,对术后复发和放疗损伤2组定量参数进行统计对比分析。结果脑星形细胞瘤术后复发组17例,其中Ⅲ级10例,Ⅳ级7例;平均SUVmax为1.41±0.75,L/N值为3.54±1.05。术后放射治疗损伤组12例,平均SUVmax为0.28±0.06,L/N值为0.82±0.03。上述2组数据的差异有统计学意义,P值均<0.05。结论脑星形细胞瘤术后行18 F-FLT分子显像,其定量参数指标SUVmax和L/N能够提高鉴别术后复发和放射治疗损伤的准确性,对指导临床术后再治疗有重要意义。
OBJECTIVE To evaluate the quantitative differential diagnosis value of maximum standardized uptake value (SUVmax) and lesion-to-contralateral normal brain (L/N) using 3'-deoxy-3'18 F-fluoro-thymidine (18 F-FLT) molec- ular imaging in post-operation astrocytomas. METHODS Dynamic 18F-FLT PET-CT was performed in 29 patients with post-operation astrocytomas who undertook radiotherapy. SUVmax and L/N ratio had been acquired by region of interest (ROD technology. Both parameters of recurrence and radiotherapy-induced brain injury were analyzed by statistics. RE- SULTS The average SUVmax of 18F-FLT in recurrent astrocytomas and radiotherapy-induced brain injury was 1.41±0.75 and 0.28±0. 06, respectively. The average L/N ratio of 18F-FLT in recurrent astrocytomas and radiotherapy-induced brain injury was 3.54±1.05 and 0.82 ± 0.03, respectively. There was significant difference between two groups (P〈 0.05). CONCLUSIONS SUVmax and L/N ratio of 18 F-FLT molecular imaging can be great helpful in the differential diag- nosis of recurrent astrocytoma and radiotherapy-induced brain injury. It is very important for the retreatment of post-op- eration astrocytoma.
