肝功能指标及慢性肝炎病毒感染状态对肝脏摄取18F-FDG的影响 被引量:7

摘要 目的探讨肝功能指标及慢性肝炎病毒感染状态对肝脏摄取18F-FDG的影响。方法回顾性分析713名(男504名,女209名,年龄24—74岁)体格检查者的18F.FDGPET/CT检查、肝功能及肝炎病毒检验资料,测量肝右叶SUVmax,通过两样本t检验比较HBsAg及HCV抗体阴性、阳性组间SUVmax。的差异,通过相关分析、偏相关分析和多元线性回归分析探讨肝功能指标与SUVmax。间相关性。结果受检者的肝脏SUVmean。为1.86±0.38。受检者中,449名有HBsAg检测结果,其中65名(14.5%)阳性,384名(85.5%)阴性;477名有HCV抗体检测结果,其中53名(11.1%)阳性,424名(88.9%)阴性。HBsAg及HCV抗体阴性、阳性组间SUVmean。差异无统计学意义(t=1.042和1.283,均P〉0.05)。结合胆红素、球蛋白、AST与SUVmax。显著相关(偏相关r'=0.191、-0.087和0.132,均P〈0.05),是预测肝脏18F—FDG摄取变化的独立因素,其中球蛋白的预测能力最大(标准化β′=0.112,P〈0.05)。结论肝脏摄取18F-FDG受某些肝功能指标影响,结合胆红素、球蛋白及AST是预测肝脏18F—FDG摄取变化的独立因素。对肝脏代谢活性的评估需结合肝功能检验结果。 Objective To identify impacts of infection status of chronic hepatitis viruses and bio- chemical parameters of liver function on hepatic FDG uptake. Methods The results of 18F-FDG PET/CT, liver function biochemical tests and hepatic virus tests of 713 healthy adults (504 males, 209 females, age 24-74 years) were retrospectively analyzed. Two-sample t test, correlation analysis, partial correlation analysis and muhiple linear regression were performed to identify correlations of liver SUV with the biochemi- cal parameters and hepatic viral infection status. Results HBsAg tests were performed in 449 subjects, with the results of 65 (14.5%) positive and 384 (85.5%) negative. HCV antibody tests were performed in 477 subjects, with 53 (11.1% ) positive and 424 (88.9%) negative. The average SUVmean of the total sub- jects was 1.86±0.38. There was no significant difference of SUV between HBsAg positive group and nega- tive group (t= 1. 042, P〉0.05), or between HCV antibody positive group and negative group (t= 1. 283, P〉 O. 05 ). Serum conjugated bilirubin, globulin and AST were significantly correlated with liver SUVmean(r' =-0.191, -0.087 and 0.132, all P〈0.05). These parameters were independent variables on predicting variance of liv- er SUV with globulin showing the largest predicting value (standardized β'=-0.112, P〈0.05 ). Con- clusions Functional status of liver may affect liver 18F-FDG uptake. Serum conjugated bilirubin, globulin and AST may be the independent variables on predicting variance of liver SUV . Liver functional test re- suits should be taken into consideration when hepatic 18F-FDG uptake is interpreted.
