摘要
目的 观察非超声乳化改良小切口白内障摘除人工晶状体植入联合小梁切除术在白内障合并青光眼临床治疗中的应用效果及其临床价值.方法 选择闻喜县人民医院2011年5月至2013年12月收治的80例白内障合并青光眼患者作为研究对象,应用非超声乳化改良小切口白内障摘除人工晶状体植入联合小梁切除术进行治疗,比较手术前后患者眼压及视力变化情况,记录并发症发生率情况.结果 治疗前患者视力情况多集中在0.1以下及0.1～0.2,所占比例分别为41.2％ （33/80）、50.0％ （40/80）,经治疗后视力得到大幅提高,主要集中在0.3 ～0.6,所占比例为68.8％（55/80）,前后对比差异有统计学意义（P＜0.05）.80例患者中7例患者前房出现纤维渗出,3例瞳孔不圆,2例眼压偏高,7例出现角膜轻度水肿,总发生率为23.7％ （19/80）.结论 应用非超声乳化改良小切口白内障摘除人工晶状体植入联合小梁切除术治疗白内障合并青光眼,可有效减轻手术创伤及术后角膜水肿情况,且快捷、方便,避免散光问题,并发症发生率低,安全系数高,费用低廉,减轻了患者经济负担,患者更易于接受,是一种理想手术方式,临床价值大,效果显著,值得推广应用.
Objective To observe the application effect and clinical value of non phacoemulsification small incision cataract extraction and modified intraocular lens implantation combined with trabeculectomy on cataract combined with glaucoma.Methods From May 2011 to December 2013,80 cases of cataract combined with glaucoma in the people＆#39; s hospital of Wenxi were chose as the research objects.Application of improved small incision non phacoemulsification cataract extraction and intraocular lens implantation combined with trabeculectomy,IOP and visual acuity of patients before and after were compared,and the complication rate was recorded.Results Before treatment in patients with visual acuity and more concentrated in the following 0.1 and 0.1-0.2,the percentage was 41.2% （33/80）,50% （40/80）,after treatment,visual acuity improved greatly,mainly concentrated in 0.3-0.6,the proportion of 68.8% （55/80）,the difference was significant（P 〈 0.05）.Among the 80 patients,7 patients with anterior chamber fibrin exudation,3 cases of pupils round,2 cases of high intraocular pressure,corneal edema occurred in 7 cases,the total incidence was 23.7% （19/80）.Conclusions The application of non phacoemulsification small incision cataract extraction with modified intraocular lens implantation combined with trabeculectomy in the treatment of cataract combined with glaucoma can effectively lessen surgical trauma and postoperative corneal edema,quick and convenient,avoid the problem of astigmatism,has low complication rate,high safety coefficient,low cost,reduce the economic burden of the patients,more easy to accept an ideal surgical methods and clinical value,is worthy of promotion and application.
