摘要
目的观察北京市儿童A组链球菌(GAS)对目前常用抗菌药物的敏感性,为临床用药提供依据。方法 2013年的5月-7月,在北京市36家医院收集儿童A组链球菌153株,全部菌株来自咽拭子标本。采用VITEK-2全自动微生物分析系统及GPS-67药敏卡进行药敏试验,依据CLSI 2012年M100-S22判定标准判断其对相关抗菌药物的敏感性。结果 153株GAS对青霉素、氨苄西林、左氧氟沙星、替加环素、万古霉素、链阳霉素、利奈唑胺敏感率均为100.0%(153/153);对克林霉素、红霉素耐药率均为99.3%(152/153);对四环素耐药率为88.2%(135/153);在不同地区、不同年龄和临床诊断病例间菌株对红霉素、克林霉素和四环素耐药率的分布差异无统计学意义(P>0.05)。结论 2013年北京市儿童A组链球菌的病原菌GAS对青霉素等β-内酰胺类和左氧氟沙星等喹诺酮类药物具有高度敏感性,对红霉素等大环内酯类抗菌药物和克林霉素耐药率高,提示临床谨慎选择该类药物。青霉素类仍然是治疗猩红热的首选药物。
Objective To observe susceptibility of group A streptococcal( GAS) from children in Beijing to the current common antibiotics,so as to provide evidences for clinical practice. Methods A total of 153 strains of Streptococcus pyogenes were collected in 2013 from patients in 36 children's hospitals in Beijing. All strains were isolates from throat swabs,which were tested by VITEK- 2 automated microbial analysis system and GPS- 67 Minka drug susceptibility card. Drug susceptibility was determined based on criteria in M100- S22,CLSI 2012. Results All the 153 isolates were susceptible to penicillin,ampicillin,levofloxacin,tigecycline,vancomycin,streptogramin,and linezolidwith the rate of 100. 0%( 153 /153); resistance rates to clindamycin,erythromycin was 99. 3%( 152 /153),that to tetracycline was 88. 2%( 135 /153),respectively. The differences of resistance rates of all strains to erythromycin,clindamycin and tetracycline in different regions,age groups and clinical diagnosis were not statistically significant(P〈0. 05). Conclusion This study demonstrated that the pathogen of group A streptococcal( GAS) from children in Beijing was highly sensitive to β- lactam penicillin and fluoroquinolones such as levofloxacin,and it has a highly resistance rate to Erythromycin and other macrolide antibacterial drugs as well as clindamycin,indicating that these drugs should be carefully selected in clinical practice. Penicillin is still the prime choice for scarlet fever.
