摘要
目的探讨新疆三种药食同源食材复配临床营养制剂对重症肺炎患者肠黏膜屏障、血清炎性因子及氧合指数的影响。方法选择新疆维吾尔自治区人民医院重症监护室2014年7月至2015年1月收治的行机械通气的重症肺炎患者54例,按照随机数字表随机分为对照组和观察组。两组患者均给予基础治疗加营养治疗。在营养支持方面,对照组给予常规营养治疗,观察组在常规营养治疗的基础上加用新疆三种药食同源食材复配营养制剂,20g/d。观察两组患者治疗前及治疗后7、14d外周血二胺氧化酶(DAO),血清炎性因子(IL-6),氧合指数(PaO2/FiO2)的变化。结果两组患者治疗后外周血DAO、IL-6水平较治疗前下降(P<0.01),观察组DAO、IL-6水平较对照组下降显著(P<0.05);两组患者治疗后PaO2/FiO2均较治疗前显著提高(P<0.01),观察组PaO2/FiO2改善情况优于对照组(P<0.05)。治疗结束,两组患者总平均机械通气时间分别为(8.92±1.86)d、(7.21±1.32)d,差异有统计学意义(P<0.05)。结论新疆三种药食同源食材复配临床营养制剂辅助治疗重症肺炎,能够保护患者的肠黏膜屏障功能,抑制炎性因子,改善通气功能,提高临床疗效。
Objective To investigate the impact of a compound enteral nutrition containing three kinds of medicinal foods from Xinjiang in patients with severe pneumonia(SP).Methods 54 patients with SP admitted in the intensive care unit(ICU)of Xinjiang Uygur Autonomous Region People's Hospital from July 2014 to January 2015 and accepted mechanical ventilation were divided randomly into the observation or control group respectively.Both groups were given basic treatment and conventional nutrition therapy,while the observation group was given additionally a compound enteral nutrition which contains three kinds of medicinal foods from Xinjiang at a dose of 20g/d.The diamine oxidase(DAO),inflammatory factor(IL-6)and oxygenation index(PaO2/FiO2)were observed in both groups before and at days 7and 14 after treatment.Results The levels of DAO and IL-6in both groups decreased after treatment(P〈0.01),which was more significant in observation group than in control group(P〈0.05).Compared with control group,the PaO2/FiO2 in observation group increased more significantly after treatment(P〈0.05).The average time of mechanical ventilation in observation group was(7.21±1.32)days versus(8.92±1.86)days in control group(P〈0.05).Conclusion This compound enteral nutrition can protect intestinal barrier function,inhibit the releasing of inflammatory factors,and improve the lung function in patients with severe pneumonia.
