摘要
目的探讨不同性别对腹部手术全身麻醉患者单次静脉注射丙泊酚的药动学影响。方法选择2012年5月-2013年5月来该院就诊并实施腹部全身麻醉手术患者36例,按照不同性别将其分为男性组和女性组,每组各18例。两组患者均采用静吸复合麻醉,分别于注射丙泊酚后的1、2、3、4、5、10、15、30、45、60、90、120、150、180、240、360和720 min时抽取3 ml静脉血,血浆丙泊酚浓度使用高效液相色谱-荧光检测法测定,并以上述时间点为时间轴绘制血药浓度-时间曲线,分别计算两组患者药代动力学参数。结果两组患者血药浓度均快速下降,但女性组腹部手术全身麻醉患者在10 min以内的各个时间点的血药浓度均显著低于男性组,差异有统计学意义(P<0.05)。10 min以后的各个时间点两组患者的血药浓度差异无统计学意义;女性组中央式分布容积(Vc)和清除率(CL)显著高于男性组,差异有统计学意义(P<0.05),两组患者的其他药代动力学参数组间比较差异无统计学意义(P>0.05)。结论对全麻患者静脉注射丙泊酚后,女性患者在各个时间点采集的血药浓度均低于男性患者,中央式分布容积(Vc)和清除率(CL)女性患者显著高于男性,表明不同性别对腹部手术全麻病人单次静脉注射丙泊酚的药代动力学有明显的影响。
【Objective】 To explore the impact of different gender on pharmacokinetics of propofol at intravenous injection for anesthesia of abdominal surgery patients. 【Methods】 A total of 36 patients who were treated with abdominal surgery under general anesthesia from May 2012 to May 2013 in our hospital were selected. According to different gender they were divided into male and female groups with 18 cases in each group. The patients in each group had intravenous inhalation anesthesia. Venous blood samples(3 ml) were taken at 1, 2, 3, 4, 5, 10, 15, 30, 45, 60, 90, 120, 150, 180, 240, 360 and 720 mins after injection of propofol. Plasma propofol concentration was measured using high performance liquid chromatography-fluorescence detection method and the plasma concentration-time curve was drawn. The pharmacokinetic parameters of both groups were calculated. 【Results】 In both groups, blood drug concentration decreased rapidly, while the blood concentration of propofol at each time point within 10 min in the female group was significantly lower than that in the male group(P〈0.05). There were no significant differences in blood drug concentrations at various time points after 10 min between both groups. The central volume of distribution of propofol(Vc) and clearance rate(CL) in the female group were significantly higher than those in the male group with statistically significant differences(P〈0.05). There were no statistically significant differences in other pharmacokinetic parameters between both groups(P〉0.05). 【Conclusions】 Different sex has significant impact on pharmacokinetics of single intravenous propofol injection for general anesthesia in patient receiving abdominal surgery.
