普外科医院感染患者的病原菌分布与耐药性研究 被引量:3

Distribution and drug resistance of pathogens causing nosocomial infections in patients of general surgery department

摘要 目的探讨普外科医院感染患者的病原菌分布与耐药性,旨在为抗菌药物的合理使用提供依据。方法回顾性分析2010年1月-2014年12月86例普外科医院感染患者的临床资料,分析其病原菌分布及耐药性,采用SPSS17.0软件对数据进行统计分析。结果感染部位以下呼吸道为主,占32.5%,其次为胃肠道、手术切口和泌尿道,分别占19.8%、16.3%和12.8%;86例普外科感染患者共分离出病原菌114株,其中革兰阴性菌78株占68.4%;革兰阳性菌25株占21.9%;真菌11株占9.7%;大肠埃希菌对亚胺培南敏感,铜绿假单胞菌对阿米卡星、亚胺培南、头孢噻肟、头孢吡肟、头孢西丁、头孢他啶、头孢呋辛敏感,肺炎克雷伯菌对氨曲南、头孢噻肟、头孢吡肟、头孢他啶敏感;革兰阳性菌对利奈唑胺、莫西沙星、万古霉素和替加环素敏感。结论普外科患者医院感染以革兰阴性菌为主,病原菌耐药性十分普遍,应根据药敏试验合理应用抗菌药物,降低病原菌耐药率。 OBJECTIVE To explore the distribution and drug resistance of pathogens causing nosocomial infections in the patients of department of general surgery so as to provide guidance for reasonable use of antibiotics.METHODS The clinical data of 86 patients with nosocomial infections who were treated in the department of general surgery from Jan 2010 to Dec 2014 were retrospectively analyzed.The distribution and drug resistance of the pathogens were observed,and the statistical analysis of data was performed with the use of SPSS17.0software.RESULTS Of the 86 patients with nosocomial infections,32.5% had lower respiratory tract infections,19.8% had gastrointestinal tract infections,16.3% had surgical incision infections,and 12.8% had urinary tract infections.A total of 114 strains of pathogens were isolated,including 78(68.4%)strains of gram-negative bacteria,25(21.9%)strains of gram-positive bacteria,and 11(9.7%)strains of fungi.The Escherichia coli was susceptible to imipenem;the Pseudomonas aeruginosa was susceptible to amikacin,imipenem,cefotaxime,cefepime,cefoxitin,ceftazidime,and cefuroxime;the Klebsiella pneumoniae was susceptible to aztreonam,cefotaxime,cefepime,and ceftazidime.The gram-positive bacteria were susceptible to linezolid,moxifloxacin,vancomycin,and tigecycline.CONCLUSIONThe gram-negative bacteria are dominant among the pathogens causing the nosocomial infections in the patients of department of general surgery.The drug resistance of the pathogens is very common,and it is necessary to reasonably use antibiotics based on the results of the drug susceptibility testing so as to reduce the drug resistance rates.
