摘要
目的:评价RTD石英纤维桩修复前牙残根残冠的临床效果。方法:对45例68颗已行完善根管治疗的前牙残根残冠进行石英纤维桩加全瓷冠修复。根据牙体预备后牙本质肩领的高度将患牙分为3组:A组高度≥2 mm;B组高度<2 mm但≥1 mm;C组高度<1 mm。治疗完成后1、6、12、24个月复查,对修复体完整性、边缘适合性、牙周及根尖周健康状况进行评价。结果:95.59%的修复体保持完整。2例修复体发生桩脱位;1例发生桩折断;其余修复体边缘适合性良好,牙周健康。C组余留牙体无箍效应,较A组修复失败率增加(P<0.05)。结论:RTD石英纤维桩具有良好的美学效果,生物相容性好且不易造成根折;但其修复效果除了和牙本质肩领的高度有关,还和余留牙本质的性状有关。在符合适应证的情况下石英纤维桩是残根残冠修复的理想选择。
Objective:To evaluate the clinical efficacy of RTD quartz fiber post for restoration of anterior residual roots and crowns. Methods: A total of 68 anterior residual roots and crowns from 45 cases, which had undergone root canal therapy, were restored with RTD quartz fiber posts and all-ceramic crowns. According to the height of dentin shoulder collar after tooth prepa- ration, the teeth were divided into three groups, group A(the height of dentin shoulder collar was ≥2 ram), group 13(the height of dentin shoulder collar was 〈2 mm but ≥1 ram) and group C(the height of dentin shoulder collar was 〈1 ram). The restora tion inlegraty, the marginal adaptation, and the periodontal and periapical status were evaluated at 1,6,12 and 24 month after the completion of treatment. Results: The integrity was maintained in 95. 59% restorations. In 2 cases, the posts were dis lodged. And in 1 case,the post was fractured. The marginal adaptation, as well as the periodontal healthy status, of the other restorations was good. The rate of restoration failure in group C was significantly higher than that in group A due to the ab sence of ferrule effect(P〈0. 05). Conclusions: RTD quartz fiber post showed excellent aesthetic effect, good biocompatibility and less prone to fractures. However, its restoration effect was related with not only the height of dentin shoulder collar, but also the characteristics of residual dentin. Thus, qruartz fiber post, in compliance with the indication, is an ideal choice for res toration of residual roots and crowns.
