摘要
目的探讨淋巴细胞参数LYMPH-Y在EB感染的传染性单核细胞增多症(IM)患儿外周血中的变化,并分析其与异型淋巴细胞、EB病毒载量的关系,以快速诊断疾病。方法用Sysmex-2100系列血球分析仪检测48例EB病毒感染IM患儿(IM组)、50名健康体检儿童(对照组)外周血标本,统计分析淋巴细胞总数(TCL)、淋巴细胞参数LYMPH-Y结果,人工镜检IM组外周血异型淋巴细胞并计算百分比,实时荧光定量PCR法检测其外周血EB-DNA含量;分析IM患儿外周血LYMPH-Y参数与异淋百分比、EB-DNA含量之间的相关性。结果 IM组淋巴细胞总数(6.58±2.52)×109/L及淋巴细胞LYMPH-Y参数71.59±9.14均高于对照组的(3.23±1.20)×109/L和57.60±4.72,差异有统计学意义(P<0.05);IM组淋巴细胞LYMPH-Y参数与异型淋巴细胞百分比呈显著正相关(r=0.693,P<0.01)、淋巴细胞LYMPH-Y参数与EB病毒载量呈显著正相关(r=0.783,P<0.01);在EB感染IM患儿的诊断中,当LYMPH-Y参数的临界值(cut-off值)取60.85时,敏感度和特异度分别为93.8%、76.0%。结论 EB病毒感染的IM患儿外周血淋巴细胞参数LYMPH-Y明显升高,且与其异型淋巴细胞百分比、EB病毒载量均呈显著正相关,可作为IM诊断辅助指标之一。
OBJECTIVE To investigate the change and the clinical significance of the lymphocyte parameter LYMPH-Y in Epstein-Barr virus-associated infectious mononucleosis(EBV-associated IM)and explore the relationship of LYMPH-Y with atypical lymphocytes and EBV load so as to rapidly diagnose diseases.METHODS The Sysmex-2100 automatic blood analyzer was applied to detect peripheral blood samples of 48 patients with EBV-associated IM(the IM group)and 50 healthy children(the control group).Results on the total count of lymphocytes(TCL)and the parameter LYMPH-Y were statistically analyzed.The atypical lymphocytes in peripheral blood of the IM group were detected by microscope and the percentage was calculated.EB-DNA load in peripheral blood was detected by real-time PCR.The correlation of the parameter LYMPH-Y with atypical lymphocytes and EBV load was analyzed.RESULTS The TCL and LYMPH-Y in the IM group(6.58±2.52×10^9/L,71.59±9.14)were significantly higher than in the control group(3.23±1.20×10^9/L,57.60±4.72)(P〈0.05).The lymphocyte LYMPH-Y parameter and the percentage of atypical lymphocytes were significantly positively related in the IM group(r=0.693,P〈0.01),and the LYMPH-Y parameter and the EB viral load were significantly positively correlated(r=0.783,P〈0.01)as well.In the diagnosis of EBV-associated IM by LYMPH-Y parameter,when the threshold(cut-off value)was 60.85,the sensitivity and specificity were 93.8% and 76.0% respectively.CONCLUSION The LYMPH-Y parameter is increased significantly in EBV-associated IM children. The LYMPH-Y parameter is positively related to atypical lymphocytes and EBV load.It can be regarded as an early assistant diagnostic indicator of EBV-associated IM.
