基质辅助激光解析电离飞行时间质谱仪在侵袭性真菌感染病原真菌鉴定中的应用 被引量:6

摘要 目的:评价基质辅助激光解析电离飞行时间质谱仪(MALDI-TOF MS)对侵袭性真菌感染病原真菌鉴定的应用价值。方法:选取2013年1月至2014年3月南方医科大学珠江医院住院部35例静脉血或脑脊液培养真菌阳性标本,用基质辅助激光解析电离飞行时间质谱(MALDI-TOF MS)技术进行鉴定,同时进行Vitek2全自动微生物鉴定系统鉴定,将两种方法的鉴定结果进行比对,两者鉴定结果不符合的采用核糖体RNA内转录间隔区2(ITS 2)序列分析最后确证结果,从而评价质谱法的准确性、实用性。结果:MALDITOF MS正确鉴定出其中的30例(85.7%),错误5例(14.3%);Vitek2全自动微生物鉴定系统正确鉴定出35例(100.0%)。结论:MALDI-TOF MS相对于传统的生化鉴定具有操作简便、快速、成本低、重复性好的优点,但本次研究的准确率不如传统生化鉴定,也比之前国内外报道的准确率低,可能跟标本的前处理难以统一规范以及蛋白提取程序不一致有关。相信随着MALDI-TOF MS技术的统一规范和图谱数据库不断的完善,基质辅助激光解析电离飞行时间质谱仪将成为日常临床微生物鉴定的理想工具。
