摘要
目的:探讨急诊腹部损伤采用多层螺旋CT平扫诊断意义。方法:本次选取腹部创伤50例,均为我院急诊科2012年7月至2014年6月收治,采用多层螺旋CT平扫,回顾临床资料。结果:本次50例患者,采用CT平扫,35例初诊阴性,阳性15例。50例中,经剖腹探查6例,1例介入治疗后完成CT随访,CT随访33例,初诊阴性者10例,经随访和住院观察,腹部无相关体征或症状,认为腹部脏器无损伤,未作深入检查。与手术治疗结果对比,48例相符,CT确诊率为96%。结论 :腹部创伤诊断中,多层螺旋CT为质的飞跃,扫描时间显著缩短,争取了急诊患者急救时间,并明显减少了运动伪影,肠道准备即使准备欠充分,重建薄层,无对比剂口服的平扫下,脏器间关系也可中较小区分,对各脏器损伤细微观察,发挥较好的诊断作用。
Objective： To investigate the use of muhislice spiral CT scans diagnosis significance of emergency abdominal injury. Methods： We selected 50 cases of abdominal trauma, all is 2012 in emergency department of our hospital from July to 2014 June was analyzed, using multi-slice spiral CT scan and retrospective. Results： The 50 eases of patients, with CT scan, 35 cases of newly diagnosed negative and positive in 15 cases. In 50 cases, 6 cases with laparotomy, CT follow-up after completion of interventional therapy of 1 cases, CT 33 cases were followed up, 10 patients with newly diagnosed negative, were followed up and stay in the hospital for observation, abdominal signs or symptoms of abdominal organs, that no damage, no in-depth examination. And the result of surgical treatment of 48 cases of contrast, consistent with CT diagnosis rate was 96%. Conclusion： The diagnosis of abdominal trauma, multi-slice spiral CT is a qualitative leap, scan time is obviously shortened, strive for the first- aid time of emergency patients, and significantly reduce motion artifacts, bowel preparation even ready to less full, reconstruction of thin, no oral contrast agent of plain, the relationship between the smaller organs can also distinguish between, subtle observation of the organ injury, play the role of a good diagnosis.
