摘要
目的探讨紫外光-核黄素胶原交联疗法治疗真菌性角膜溃疡的有效性。方法对2014年7月至12月在郑州视光眼科医院就诊的8例真菌性角膜溃疡患者,在综合药物治疗至少1周以上无效或效果欠佳的情况下给予药物联合紫外光-核黄素胶原交联疗法。用裂隙灯检查患者的角膜变化情况,追踪访问其主观感受,用共聚焦显微镜观察真菌菌丝并检测角膜各层细胞的形态变化。结果 7例病变深及前中部基质层的患者术后溃疡灶愈合;1例病变深及深基质层者病情未得到有效控制。所有患者未见手术相关并发症。结论对于综合药物治疗无效或效果欠佳的真菌性角膜溃疡,使用紫外光-核黄素胶原交联疗法治疗有明显临床效果。
Objective To investigate the efficiency of ultraviolet A-riboflavin cor- neal collagen cross-linking （CXL） in treatment of fungal corneal ulcer. Methods Eight patients with fungal corneal in our hospital from July 2014 to December 2014 were treated with CXL combining with medications after poor therapeutic effects of compre- hensive therapy with only medications. The corneal status was observed with slit-lamp, the corneal cells were examined with confocal microscopy and patients＂ subjective feel- ings were recorded. Results The conjunctival hyperaemia in 7 patients were relieved, and their ulcers were healed after the operation. 1 patient with the d∽ep matrix layer in- fected didn＇ t recover after the operation. No postoperative complication was found. Conelusion CXL is effective for fungal corneal ulcer,which can not achieve ideal ef- ficacy with the treatment of medications.
