摘要
目的 观察云南白药联合珍珠粉对老年痴呆并发应激性溃疡出血患者的临床止血和溃疡修复治疗效果,选择能避免质子泵抑制剂因抑制胃酸而降低老年患者胃肠道消化吸收和防御功能方面副作用的更加可靠的治疗方案。方法 于2012年1月-2014年12月对51例老年痴呆并发SUB患者进行前瞻性随机对照研究。其中,观察组（26例）予以云南白药加珍珠粉,对照组（25例）予以奥美拉唑。分阶段行实验室检验指标和胃镜检查评价疗效。结果 同组治疗后实验室检验结果比较,Hb、胃液p H、胃液OB、大便OB,差异有统计学意义（P〈0.05）;两组间比较,差异无统计学意义（P〉0.05）。两组止血效果总有效率分别为96.15%和96.00%（P〉0.05）;内镜下观察,两种方法止血效果和黏膜修复上比较差异无统计学意义（P〉0.05）。结论 云南白药联合珍珠粉治疗老年痴呆并发SUB患者,在出血和溃疡修复上与奥美拉唑的疗效相当,但云南白药加珍珠粉治疗老年痴呆并发SUB,既有利于促进SUB康复,也有利于增加机体营养,促进睡眠改善,更适合老年患者的治疗,值得临床推广应用。
Objective To observe the repair treatment effect of Yunnan Baiyao combined with pearl powder on Alzheimer＇s disease complicated with stress ulcer hemorrhage in patients with bleeding and ulcer, and avoid the proton pump inhibitor for inhibiting gastric acid and reduce more reliable therapeutic regimens for elderly patients with gas- trointestinal digestion absorption function and defensive side effects were observed. Methods A prospective randomized controlled study of 51 elderly patients with dementia complicated with SUB from January 2012 to December 2014 was conducted. Among them, the observation group （26 cases） of Yunnan Baiyao plus pearl powder, the control group （25 cases） treated with omeprazole. The clinical effect of laboratory test and gastroscopy was evaluated in stages. Results Comparison of results of laboratory examination after treatment in the same group, there were significant difference be- tween Hb, pH of gastric juice,gastric juice OB and stool OB（P〈0.05）, there was no statistically significanct difference between the two groups （P〉0.05）, compared the total effective rate of hemostatic effect 96.15% and 96.00%（P〉0.05）; Under endoscopic observation, the difference of two methods hemostatic effect and mucosal healing without statistical significance（P〉0.05）. Conclusion Yunnan Baiyao combined with pearl powder treatment in patients with senile demen- tia of concurrent sub, in repair of bleeding and ulcers and omeprazole in the treatment of quite, but Yunnan Baiyao plus pearl powder treatment senile dementia concurrent sub, is conducive to promoting the SUB the rehabilitation and to increase the body＇s nutrition, promote sleep to be better, more suitable for the treatment of elderly patients,it is worthy of clinical application.
