摘要
目的观察补髓活血健骨汤联合髓芯减压植骨术治疗非创伤性股骨头缺血性坏死(NONFH)的临床疗效。方法将89例NONFH患者随机分为2组。对照组44例,单纯予髓芯减压植骨术治疗;治疗组45例,予补髓活血健骨汤联合髓芯减压植骨术治疗。2组均随访1年,观察2组治疗1年后X线摄片及Ficat分期变化,并统计影像学疗效;采用Harris髋关节功能评分标准评价髋关节功能恢复情况及临床疗效,包括疼痛程度、生活能力、关节畸形及关节活动度评分,并分别统计治疗组Ficat分期Ⅱ期、Ⅲ期患者临床疗效。结果治疗组影像学疗效显效率及总有效率均高于对照组(P<0.05);治疗组临床疗效临床痊愈率及总有效率均高于对照组(P<0.05);2组治疗后Harris各项评分及总分与本组治疗前比较均有增加,差异均有统计学意义(P<0.05),其中治疗组疼痛程度、生活能力及总分治疗后增加多于对照组(P<0.05);治疗组Ficat分期Ⅱ期临床治愈率高于Ⅲ期(P<0.05)。结论补髓活血健骨汤联合髓芯减压植骨术治疗NONFH临床疗效确切,可明显促进股骨头修复,提高Harris评分,改善髋关节功能,且对Ficat分期Ⅱ期临床疗效优于Ⅲ期。
Objective To investigate the clinical effect of Busui - Huoxue - Jiangu decoction combined with bone grafting pith decompression on the treatment of nontraumatic avascular necrosis of the femoral head （NONFH）. Methods 89 NONFH patients order of hospitalization were randomly divided into two groups. 44 patients in control group were treated by simple bone grafting pith decompression. 45 patients in treatment group were treated by Busui - Huoxue - Jiangu decoction combined with bone grafting pith decompression. Patients in two groups were followed up for one year. The radiography and Ficat change in stages were observed after one year, and the imaging efficacy was evaluated. The recovery condition of hip joint function and curative effect were evaluated by Harris hip joint function scale, including the score of pain degree, viability, joint deformity and the range of motion. The curative effects of Ficat stage Ⅱ and Ⅲ in treatment group were analyzed respectively. Results The obvious effective rate and total effective rate of imaging efficacy in treatment group were superior to those in control group （ P 〈 0.05 ）. The clinical cure rate and clinical total effective rate in treatment group were superior to those in control group （ P 〈 0.05 ）. There were significant differences between before and after treatment on the each item and total score of Hart＇is hip joint function scale in two groups （ P 〈 0.05）, the score was increased, and the increase of pain, viability and total score in treatment group were superior to those in control group （P 〈 0.05 ）. The Ficat stage Ⅱ had better clinical cure rate as compared with Ficat stage Ⅲ in treatment group （ P 〈 0.05 ）. Conclusion Combined therapy of Busui - Huoxue -Jiangu decoction and bone grafting pith decompression has an exact clinical effect on NONFH, can obviously promote the renovation of femoral head necrosis, increased the score of Harris hip joint function scale, improve the function of hip joint, and has a better clinical effect on Ficat stage Ⅱ.
